目的:探讨血液红系参数和胱抑素 C(CysC)联合检测在多发性骨髓瘤患者贫血病变程度判定中的价值。方法选取60例多发性骨髓瘤并贫血的初发住院患者,根据血红蛋白(HGB)水平不同分组,HGB≤60 g/ L 患者共21例(A 组),HGB 61~89 g/ L 患者23例(B 组),HGB 90~110 g/ L 患者16例(C 组)。分析各组血液红系参数平均血清血红蛋白(MCH)、平均血球血红蛋白浓度(MCHC)、平均红细胞容积(MCV)、红细胞分布宽度(RDW)及 CysC 水平,观察上述各指标与 HGB 的相关性。结果 A 组、B 组 HGB 低于 C 组,RDW、CysC 水平高于 C 组( P ﹤0.05),A 组 HGB 低于B 组( P ﹤0.05);RDW、CysC 阳性率 A 组为85.71%、90.48%,B 组为78.26%、82.61%,C 组为25.00%、18.75%,A 组、B 组 RDW、CysC 阳性率高于 C 组阳性率( P ﹤0.05)。患者 HGB 与 RDW、CysC 水平间 Pearson 相关性分析呈负相关( P ﹤0.05)。结论对多发性骨髓瘤初发患者检测血液红系参数 RDW 及 CysC 有助于了解患者贫血病变程度变化,贫血改变与 RDW、CysC 升高关系密切。

목적:탐토혈액홍계삼수화광억소 C(CysC)연합검측재다발성골수류환자빈혈병변정도판정중적개치。방법선취60례다발성골수류병빈혈적초발주원환자,근거혈홍단백(HGB)수평불동분조,HGB≤60 g/ L 환자공21례(A 조),HGB 61~89 g/ L 환자23례(B 조),HGB 90~110 g/ L 환자16례(C 조)。분석각조혈액홍계삼수평균혈청혈홍단백(MCH)、평균혈구혈홍단백농도(MCHC)、평균홍세포용적(MCV)、홍세포분포관도(RDW)급 CysC 수평,관찰상술각지표여 HGB 적상관성。결과 A 조、B 조 HGB 저우 C 조,RDW、CysC 수평고우 C 조( P ﹤0.05),A 조 HGB 저우B 조( P ﹤0.05);RDW、CysC 양성솔 A 조위85.71%、90.48%,B 조위78.26%、82.61%,C 조위25.00%、18.75%,A 조、B 조 RDW、CysC 양성솔고우 C 조양성솔( P ﹤0.05)。환자 HGB 여 RDW、CysC 수평간 Pearson 상관성분석정부상관( P ﹤0.05)。결론대다발성골수류초발환자검측혈액홍계삼수 RDW 급 CysC 유조우료해환자빈혈병변정도변화,빈혈개변여 RDW、CysC 승고관계밀절。
Objective To explore the combined detection of blood erythroid parameters and cystatin C(CysC)in observation of the severi-ty of anemia in patients with multiple myeloma,in order to provide the reference fordifferential diagnosis of multiple myeloma and severity of anemi-a. Methods A total of 60 cases of multiple myeloma and anemia in hospitalized patients were divided into 2 groups depending on the level of he-moglobin(HGB),21 patients with HGB 60 g/ L or less were allocated in group A,and 23 patients with HGB 61 ~ 89 g/ L in group B,16 patients with HGB 90 ~ 110 g/ L in group C,and the analysis of red blood cell parameters as MCH,MCHC,MCV,RDW and change in serum level of Cy-sC,observe the correlation of the respective index and HGB. Results The level of HGB in groups A and B waslower than that of group C,the lev-els of RDW and CysC in group A and group B were higher than those of group C( P ﹤ 0. 05),the level of HGB in group A was lower than that of group B( P ﹤ 0. 05);the positive rates of RDW andCysC in group A,group B and group C were 85. 71% and 90. 48% ,78. 26% and 82. 61% , 25. 00% and 18. 75% respectively. The positive rates of RDW and CysC in group A and group B were higher than those of group C( P ﹤ 0. 05);and Pearson correlation coefficient analysis on levels of RDW and CysC showed there was a negative correlation between them( P ﹤ 0. 05). Con-clusion In patients with incipient multiple myeloma,the measure of red blood parameters as RDW and change in level of CysC contributes to un-derstand the severity of anemia in patients with the degree of pathological changes,and the change in anemia is closely correlated with the increase of RDW andCysC.
