目的：探讨急性脑卒中早期（72小时内）死亡的主要相关危险因素，降低早期死亡率。方法：收集我院2004年1月至2012年12月经急诊科抢救诊断为急性脑卒中的242例病患，根据纳入标准，分为死亡组和对照组，采用回顾性对照分析方法，对相关数据进行统计分析。结果：急性脑卒中患者发生72 h内死亡的影响因素显示中风类型、年龄、高血压病史、心脏病病史、脉搏、呼吸、合并症、中线移位、意识状态、GCS评分、WBC、C反应蛋白、PT、APTT 活化部分凝血酶时间、D－II聚体、LDL、LDH、CK、CK－MB、血糖、BUN、Cr均与72 h内死亡密切相关。中风类型、年龄、合并症、GCS评分、WBC、C反应蛋白、LDL、血糖是引起脑卒中发病72 h内死亡的独立危险因素。结论：中风类型、年龄、合并症、GCS评分、WBC、C反应蛋白、LDL和血糖是脑卒中发病早期死亡的独立危险因素，临床应予充分重视。
목적：탐토급성뇌졸중조기（72소시내）사망적주요상관위험인소，강저조기사망솔。방법：수집아원2004년1월지2012년12월경급진과창구진단위급성뇌졸중적242례병환，근거납입표준，분위사망조화대조조，채용회고성대조분석방법，대상관수거진행통계분석。결과：급성뇌졸중환자발생72 h내사망적영향인소현시중풍류형、년령、고혈압병사、심장병병사、맥박、호흡、합병증、중선이위、의식상태、GCS평분、WBC、C반응단백、PT、APTT 활화부분응혈매시간、D－II취체、LDL、LDH、CK、CK－MB、혈당、BUN、Cr균여72 h내사망밀절상관。중풍류형、년령、합병증、GCS평분、WBC、C반응단백、LDL、혈당시인기뇌졸중발병72 h내사망적독립위험인소。결론：중풍류형、년령、합병증、GCS평분、WBC、C반응단백、LDL화혈당시뇌졸중발병조기사망적독립위험인소，림상응여충분중시。
Objective: To explore the main associated risk factors of the early death (within 72 hours) in acute brain stroke, in order to reduce the early mortality. Methods: 242 cases of acute brain stroke patients which diagnosed in the emergency department between January 2004 and December 2012 were analyzed. These cases were divided into the death group and the control group by the entry standards. A retrospective comparative analysis method was used to analysis this relevant data. Results: The influence factors of the early death (within 72 hours) in acute brain stroke including types of stroke,age, hypertension, heart disease, pulse, respiration, complications, midline shift, consciousness, GCS score, the WBC,CRP, PT, APTT activation thrombin time, D - dimer II ,LDL,LDH, CK,CK-MB,blood glucose,BUN and Cr. Types of stroke,age,complications,GCS score,WBC,CRP,LDL,blood glucose were the independent risk factors for the early death(within 72 hours from the onset) in stroke. Conclusion:Types of stroke, age, complication, GCS score, the WBC, CRP, LDL and blood glucose are the independent risk factors for the early death(within 72 hours) in stroke, clinical doctors should pay more attention to these factors.
