目的:探讨脑卒中患者神经功能缺损评分与载脂蛋白的相关性。方法检测220例脑卒中患者治疗前的血浆载脂蛋白 A1(Apo A1)、血浆载脂蛋白 B(Apo B)、脑脊液载脂蛋白 E(Apo E)及血浆载脂蛋白 H(Apo H)的水平,以及神经功能缺损评分。比较各载脂蛋白前10%和后10%患者临床资料和神经功能缺损评分,并探讨载脂蛋白水平与神经功能缺损评分的相关性。结果 Apo A1前10%和后10%患者间 NIHSS 评分和梗死面积差异有统计学意义( P ﹤0.05),Apo B 前10%和后10%患者间 NIHSS 评分和梗死面积无统计学差异( P ﹥0.05),Apo E 前10%和后10%患者间NIHSS 评分和梗死面积差异有统计学意义( P ﹤0.01),Apo H 前10%和后10%患者间 NIHSS 评分无统计学差异( P ﹥0.05),血清 Apo A1水平和脑脊液 Apo E 水平均与 NIHSS 评分负相关。结论血清中 Apo A1和脑脊液中 Apo E 水平均与 NIHSS 评分负相关,可以作为判断脑卒中病情和预后的有效指标。

목적:탐토뇌졸중환자신경공능결손평분여재지단백적상관성。방법검측220례뇌졸중환자치료전적혈장재지단백 A1(Apo A1)、혈장재지단백 B(Apo B)、뇌척액재지단백 E(Apo E)급혈장재지단백 H(Apo H)적수평,이급신경공능결손평분。비교각재지단백전10%화후10%환자림상자료화신경공능결손평분,병탐토재지단백수평여신경공능결손평분적상관성。결과 Apo A1전10%화후10%환자간 NIHSS 평분화경사면적차이유통계학의의( P ﹤0.05),Apo B 전10%화후10%환자간 NIHSS 평분화경사면적무통계학차이( P ﹥0.05),Apo E 전10%화후10%환자간NIHSS 평분화경사면적차이유통계학의의( P ﹤0.01),Apo H 전10%화후10%환자간 NIHSS 평분무통계학차이( P ﹥0.05),혈청 Apo A1수평화뇌척액 Apo E 수평균여 NIHSS 평분부상관。결론혈청중 Apo A1화뇌척액중 Apo E 수평균여 NIHSS 평분부상관,가이작위판단뇌졸중병정화예후적유효지표。
Objective To explore the correlation between defect scores of nerve function and apolipoprotein in patients with cerebral apo-plexy. Methods The serum levels of lipid protein A1,apolipoprotein B,cerebrospinal fluid containing apolipoprotein E and plasma apolipopro-tein H were examined in 220 patients with cerebral apoplexy before the treatment,and the clinical data and defect scores of neural function in 10%patients were compared before and after the treatment,and the relationship between their defect scores of neural function and serum levels of apoli-poprotein were compared and evaluated. Results The difference in NIHSS scores and infarct size between serum levels of ApoA1 in 10% patients before and after treatment was statistically significant( P ﹤ 0. 05). The difference in NIHSS scores and infarct size between serum levels of ApoB before treatment and after treatment in 10% patients was not significant( P ﹥ 0. 05). The difference in NIHSS scores and infarct size between CSF levels of ApoE in 10% patients before and after treatment was statistically significant( P ﹤ 0. 01). The difference in NIHSS scores and in-farct size between serum levels of Apo H in 10% patients before and after treatment was not significant( P ﹥ 0. 05). The blood level of ApoA1 and CSF level of ApoE were negatively correlated with NIHSS scores. Conclusion The relationship between serum level of ApoA1 and CSF level of ApoE with NIHSS scores showed negative correlation,and they can be used as effective indexes for evaluation of the severity and prognosis of stroke in patients with cerebral apoplexy.
