目的:观察骨髓间充质干细胞( BMSCs)移植对脑缺血再灌注损伤脑微血管完整性的保护作用以及脑脉通颗粒对其的影响。方法体外培养和扩增BMSCs;大鼠随机分为假手术组、模型组、移植组、脑脉通组、联合组;线栓法制备MCAO模型;术后24 h将BMSCs由同侧颈内动脉移植脑内;光镜下观察BMSCs移植脑内7、14和28 d脑微血管病理改变,免疫组化法测定脑组织IgG和微血管基底膜ColⅣ及LN的表达。结果模型组大鼠IgG表达显著增强(P<0.01);与模型组比较,脑脉通和联合组各时间点、移植组14 d和28 d的IgG表达均显著减弱(P<0.01);联合组较移植组7 d和28 d、较脑脉通组14 d和28 d的表达均减弱(P<0.01)。各模型组ColⅣ和LN表达减弱(P<0.01);与模型组比较,脑脉通和联合组各时间点、移植组14 d和28 d的ColⅣ和LN表达均增(P<0.01);联合组较各移植组、14 d和28 d脑脉通组ColⅣ和LN表达均较显著增强(P<0.01)。结论脑缺血再灌注损伤引起微血管基底膜层连蛋白和胶原降解明显,血脑屏障通透性增强;BMSCs移植后早期(7 d)保护微血管受损的作用不明显,中期(14 d)和后期(28 d)用随移植脑内时间的延长而增强;脑脉通可使BMSCs移植的保护作用提前,并使中、后期的作用增强,其途径可能与减轻微血管基底膜LN和ColⅣ的降解有关。

목적:관찰골수간충질간세포( BMSCs)이식대뇌결혈재관주손상뇌미혈관완정성적보호작용이급뇌맥통과립대기적영향。방법체외배양화확증BMSCs;대서수궤분위가수술조、모형조、이식조、뇌맥통조、연합조;선전법제비MCAO모형;술후24 h장BMSCs유동측경내동맥이식뇌내;광경하관찰BMSCs이식뇌내7、14화28 d뇌미혈관병리개변,면역조화법측정뇌조직IgG화미혈관기저막ColⅣ급LN적표체。결과모형조대서IgG표체현저증강(P<0.01);여모형조비교,뇌맥통화연합조각시간점、이식조14 d화28 d적IgG표체균현저감약(P<0.01);연합조교이식조7 d화28 d、교뇌맥통조14 d화28 d적표체균감약(P<0.01)。각모형조ColⅣ화LN표체감약(P<0.01);여모형조비교,뇌맥통화연합조각시간점、이식조14 d화28 d적ColⅣ화LN표체균증(P<0.01);연합조교각이식조、14 d화28 d뇌맥통조ColⅣ화LN표체균교현저증강(P<0.01)。결론뇌결혈재관주손상인기미혈관기저막층련단백화효원강해명현,혈뇌병장통투성증강;BMSCs이식후조기(7 d)보호미혈관수손적작용불명현,중기(14 d)화후기(28 d)용수이식뇌내시간적연장이증강;뇌맥통가사BMSCs이식적보호작용제전,병사중、후기적작용증강,기도경가능여감경미혈관기저막LN화ColⅣ적강해유관。
Objective To observe the effect on integrity of brain micrangium in rats with cerebral ischemia reperfusion injury after BMSCs transplantation, as well as the role of NaoMaiTong( NMT) in the protection of BMSCs transplantation.Methods Middle cerebral ar-tery occlusion ( MCAO) model was duplicated with nylon thread.Rats of transplantation and combination groups were transplanted with BM-SCs via carotid artery at 24 h after operation.Brain micrangium pathological changes were observed by light microscope, and immunohisto-chemical method was used to determine the expressions of IgG, ColⅣand LN.Results The expressions of IgG were increased in rats brain of each model group( P<0.01 ) .Comparing with that of model group, the expression of IgG was decreased in each NMT and combination groups, as well as in 14 d and 28 d transplantation groups(P<0.01).In comparison with those of 7 d and 28 d transplantation groups and 14 d and 28 d NMT groups, IgG expressions were decreased significantly in combination groups.The expressions of ColⅣand LN were all de-creased in each model group.Comparing with those of model groups, the expressions of ColⅣand LN were increased in each NMT and com-bination groups, as well as in 14 d and 28 d transplantation groups ( P<0.01 ) .In comparison with those of 7 d and 28 d transplantation groups, 14 d and 28 d NMT groups,the ColⅣand LN expressions were increased significantly in combination groups.Conclusions NMT could make the effect of BMSCs transplantation ahead of schedule and enhance the protection in metaphase(14 d) and anaphase(28 d) , and the way might be related to soften the degradation of LN in earlier period and ColⅣin metaphase and anaphase.
