目的:评价有晶状体眼后房型人工晶状体(PPC-ICL)植入治疗高度近视术后患者水平隐斜及融合功能的变化。方法自身对照研究。被检者为行PPC-ICL植入术治疗高度近视的患者42例(84眼),分别于术前、术后1个月和术后6个月行远、近水平隐斜及远、近内外融合的测定。采用重复测量的方差分析对术前、术后1个月及6个月以上各参数进行统计分析。结果术后1个月时看远、看近水平隐斜分别为(-2.24±3.33)△和(-4.95±5.45)△,术后6个月时分别为(-2.02±2.93)△和(-3.55±5.10)△,均低于术前[(-4.33±4.62)△和(-8.88±6.74)△],差异有统计学意义(F=5.00、9.53,P<0.01)。术后1个月时看远、看近内融合分别为(12.74±3.52)△和(14.36±3.96)△,术后6个月时分别为(13.95±4.01)△和(16.79±3.90)△,均高于术前[(9.83±2.68)△和(10.95±3.28)△],差异有统计学意义(F=15.83、25.96,P<0.01)。术后1个月时看远、看近外融合分别为(8.05±2.15)△和(15.29±3.52)△,术后6个月时分别为(7.74±1.86)△和(14.64±3.11)△,均高于术前[(7.36±2.66)△和(13.43±3.11)△]。术后看近外融合较术前差异有统计学意义(F=3.53,P<0.05),看远外融合差异无统计学意义(F=0.99,P>0.05)。结论 PPC-ICL植入术后患者水平隐斜和融合范围较术前改善,视觉舒适度提高。

Objective To evaluate the changes in horizontal phoria and fusional function after phakic posterior chamber implantable contact lens (PPC-ICL) implantation for high myopia. Methods In this self controlled study, PPC-ICL implantation surgery was performed on 84 eyes of 42 high myopia patients. Far and near horizontal phoria , medial fusional range and lateral fusional range were calculated before surgery and 1 and 6 months after surgery. A variance analysis test of continuous measurement data was used for statistical analysis. Results Before surgery, far horizontal phoria was-4.33△±4.62△ and near horizontal phoria was -8.88△±6.74△. One and 6 months after surgery, it was-2.24△±3.33△ and -2.02△±2.93△ for far horizontal phoria and -4.95△±5.45△ and -3.55△±5.10△ for near horizontal phoria. All values decreased. The differences for far and near horizontal phoria were statistically significant (F=5.00, 9.53, P<0.01). One and 6 months after surgery, the far medial fusional range was 12.74△±3.52△ and 13.95△±4.01△ and the near medial fusional range was 14.36△±3.96△ and 16.79△±3.90△. These values were obviously larger than the 9.83△±2.68△ and 10.95△±3.28△ values before surgery. The differences were statistically significant (F=15.83, 25.96, P<0.01). One and 6 months after surgery, the far lateral fusional range was 8.05△±2.15△ and 7.74△±1.86△ and the near lateral fusional range was 15.29△±3.52△ and 14.64△±3.11△. These values were larger than the 7.36△±2.66△and 13.43△±3.11△ before surgery. The difference in far range was not statistically significant (F=0.99, P>0.05) while the difference in the near lateral fusional range was statistically significant (F=3.53, P<0.05). Conclusion Horizontal phoria and fusional range after PPC-ICL implantation obviously improved compared with the values before surgery and visual comfort also improved.
