目的:比较 X 线钼靶与高频超声检查在乳腺微钙化病变诊断中效能。方法收集经手术或组织学病理证实的乳腺微钙化病变患者35例为研究对象,分别进行乳腺 X 线钼靶与高频超声检查,比较高频超声、X 线钼靶、X 线钼靶+高频超声3种方法的诊断效果。结果35例乳腺微钙化病变患者中,术后经病理证实,恶性病变24例,良性病变11例;高频超声检出26例,其中恶性病变检出率为87.5%(21/24)明显高于良性检出率45.5%(5/11),差异有统计学意义(P ﹤0.05)。高频超声灵敏度、阴性预测值明显高于 X 线钼靶,差异有统计学意义(P ﹤0.05),X 线钼靶特异度、阳性预测略高于高频超声,2组比较差异无统计学意义(P ﹥0.05);高频超声+ X 线钼靶特异度、准确率、阳性预测值均明显高于高频超声、钼靶超声,且灵敏度、阴性预测值明显高于 X 线钼靶,差异有统计学意义( P ﹤0.05)。结论高频超声具有较好的灵敏度,X 线钼靶具有较好的特异度,二者联合检测更能提高检测的诊断效能。

목적:비교 X 선목파여고빈초성검사재유선미개화병변진단중효능。방법수집경수술혹조직학병리증실적유선미개화병변환자35례위연구대상,분별진행유선 X 선목파여고빈초성검사,비교고빈초성、X 선목파、X 선목파+고빈초성3충방법적진단효과。결과35례유선미개화병변환자중,술후경병리증실,악성병변24례,량성병변11례;고빈초성검출26례,기중악성병변검출솔위87.5%(21/24)명현고우량성검출솔45.5%(5/11),차이유통계학의의(P ﹤0.05)。고빈초성령민도、음성예측치명현고우 X 선목파,차이유통계학의의(P ﹤0.05),X 선목파특이도、양성예측략고우고빈초성,2조비교차이무통계학의의(P ﹥0.05);고빈초성+ X 선목파특이도、준학솔、양성예측치균명현고우고빈초성、목파초성,차령민도、음성예측치명현고우 X 선목파,차이유통계학의의( P ﹤0.05)。결론고빈초성구유교호적령민도,X 선목파구유교호적특이도,이자연합검측경능제고검측적진단효능。
Objective To compare the effectiveness of high frequency ultrasound and X-ray mammography in diagno-sis of breast lesions with micro calcification. Methods 35 cases of breast lesions were collected by surgery or pathology,and cases were examined by mammography and high frequency ultrasound. The diagnostic results were compared with the three methods of high frequency ultrasound,X-ray mammography,mammography and high frequency ultrasound. Results in 35 ca-ses of breast lesions,24 cases of malignant lesions,cases of malignant lesions,11 cases of benign lesions,26 cases of high fre-quency ultrasound,the detection rate of malignant lesions was 87. 5%(21 / 24)was significantly higher than the positive rate of 45. 5%(5 / 11)(P ﹤ 0. 05). The sensitivity and negative predictive value of high frequency ultrasound were significantly higher than those of the X-ray mammography,the difference was statistically significant(P ﹤ 0. 05),and the accuracy of the 2 groups was significantly higher than that of high frequency ultrasound(P ﹥ 0. 05). Conclusion high frequency ultrasound has good sensitivity and specificity of mammography is better,and the combined detection can improve the diagnostic efficiency.
