目的建立以社区家庭医生负责制为主体的高血压集中管理新型模式,研究该新型管理模式干预下对高血压患者低密度脂蛋白数值变化及血糖、电解质、尿常规、肾功能、血脂、胸片、心电图、眼底检查等高血压并发症8项临床指标检查率变化的影响,以探讨更为合理的高血压管理模式.方法对比新管理模式开展前即2010年与新管理模式开展后2年即2012年的社区高血压居民低密度脂蛋白数值及血糖、肾功能等8项临床指标检查率的变化.结果新管理模式干预前后,社区高血压患者低密度脂蛋白控制指标分别为(2.75±0.78)mmol/L与(2.66±0.70)mmol/L,差异有统计学意义(P<0.05).血糖、电解质、尿常规、肾功能、血脂、胸片、心电图、眼底检查等高血压并发症8项临床指标检查率差异均有统计学意义(P<0.05).结论以社区家庭医生负责制为主体的高血压集中管理模式对高血压患者低密度脂蛋白控制及高血压并发症相关临床指标参与检查率有良好的干预作用,是一种值得提倡与推广的新的先进的高血压管理模式.

Objective To establish the new hypertension centralized management mode with responsibility system of community-family physicians as the main body,and to research effect of the intervention of this new management mode on numerical changes of low density lipoprotein and 8 clinical indexes examination rate of hypertension complications including blood glucose,electrolyte,urine routine,renal function,blood lipid,chest film,electrocardiogram and fundus examination to explore a more proper hypertension management.MethodsNumerical changes of low density lipoprotein and changes of 8 clinical indexes examination rate including blood glucose,renal function and so on of community residents with hypertension before 2010 and after 2012 development of new management modes were compared.ResultsLow density lipoprotein control index of community patients with hypertension before and after interventions of new management mode were (2.75±0.78)mmol/L and (2.66±0.70)mmol/L respectively (P<0.05).The difference was highly statistically significant.Differences of 8 clinical indexes examination rate of hypertension complications including blood glucose,electrolyte,urine routine,renal function,blood lipid,chest film,electrocardiogram and fundus examination were(P<0.05) and difference was highly statistically significant.Conclusion Hypertension centralized management mode with responsibility system of community-family physicians as the main body has a good intervention effect on low density lipoprotein control and related clinical indexes examination rate of hypertension complication of hypertension patients which is a new and advanced hypertension management mode worthy of advocating and promoting.
