目的 探讨大脑白质病变严重程度与老年综合征的关系.方法 结果回顾性分析2012年2月~2014年5月间我院接受CT脑部检查老年患者的临床资料,纳入106例患者,其中男72例,女34例.平均年龄75.2岁,CT评估研究对象的WMD情况.x2检验分析WMD情况与研究对象年龄、性别、吸烟史、饮酒史、高血压史等临床资料的关系.单因素和多因素相关性分析确定WMD严重程度、WMD分布区域与老年综合征负担的关系.结果WMD的发生情况与患者的年龄、吸烟史、饮酒史有关(P<0.05),与高血压、高血脂、冠心病的病史有关(P<0.05).在6种老年病综合征中,认知障碍、跌倒、尿失禁、功能障碍与WMD的发生与否相关(P<0.05).Spearman分析显示,老年综合征的的征象个数与WMD的评分呈正相关.单因素和多因素相关分析均认为WMD的严重程度与老年综合征负担密切相关.额叶部、顶枕区以及颞叶的WMD评分与老年综合征负担密切相关(P<0.05).结论 WMD严重程度可以反映老年综合征负担,WMD情况可用于预测老年综合征的发生、评估其进展.

Objective To investigate the relationship of the severity of brain white matter disease with geriatric syndromes burden.MethodsIn this retrospective study,105 cases were selected with average age of 75.2 years old including 72 men and 34 women.All subjects have undergone brain CT scan and the severity of brain white matter disease (WMD) was estimated by two independent radiologists.The association of WMD with clinical information such as age,sex,history of smoking and drinking,vascular risk factor and so on was analyzed by Chi-square test.Regression analysis was performed to determine the relationship of the severity of WMD with geriatric syndromes burden.ResultsThe WMD presence was significantly associated with age,history of smoking and drinking and vascular risk factor.Cognitive impairment,falls and urinary incontinence were also related to WMD presence.A significant association of the severity of WMD with the geriatric syndromes burden was observed using regression analysis.Furthermore,an association between severity of WMD and geriatric syndromes burden in the frontal region,parietal-occipital region and temporal region was also present in the current study.ConclusionThere is a significant association between the severity of WMD and the geriatric syndromes burden,hence estimating the progression of WMD may contribute to predict the geriatric syndromes in aging patient.
