目的：研究3.0T磁共振DCE与DWI诊断乳腺疾病中的应用价值，以确定合理扫描序列。方法：收集60例行乳腺磁共振成像(MRI)检查并经病理证实的乳腺疾病患者，病灶62个，比较平扫+DWI、平扫+DCE及平扫+DWI+DCE三组方法诊断乳腺恶性疾病的灵敏度和特异度。结果：平扫+DWI、平扫+DCE及平扫+DWI+DCE三组扫描方法的灵敏度和特异度分别为67.6%、89%、86%和88%、84%、88%，对乳腺恶性病变检出率三组方法之间相比，差异有统计学意义(x2=9.271，P＜0.05)；平扫+DCE与平扫+DWI对乳腺恶性病变检出率二组方法之间相比差异显著，具有统计学意义(x2=6.773，P＜0.05)；平扫+DCE与平扫+DWI+DCE对乳腺恶性病变检出率二组之间相比，差异无统计学意义(x2=0.158，P＞0.05)。结论：3.0T磁共振平扫+DCE对于乳腺疾病的定性具有较高的临床价值，可作为乳腺疾病术前标准扫描序列。
목적：연구3.0T자공진DCE여DWI진단유선질병중적응용개치，이학정합리소묘서렬。방법：수집60례행유선자공진성상(MRI)검사병경병리증실적유선질병환자，병조62개，비교평소+DWI、평소+DCE급평소+DWI+DCE삼조방법진단유선악성질병적령민도화특이도。결과：평소+DWI、평소+DCE급평소+DWI+DCE삼조소묘방법적령민도화특이도분별위67.6%、89%、86%화88%、84%、88%，대유선악성병변검출솔삼조방법지간상비，차이유통계학의의(x2=9.271，P＜0.05)；평소+DCE여평소+DWI대유선악성병변검출솔이조방법지간상비차이현저，구유통계학의의(x2=6.773，P＜0.05)；평소+DCE여평소+DWI+DCE대유선악성병변검출솔이조지간상비，차이무통계학의의(x2=0.158，P＞0.05)。결론：3.0T자공진평소+DCE대우유선질병적정성구유교고적림상개치，가작위유선질병술전표준소묘서렬。
Objective:To evaluate the clinic value and advantage of DCE and DWI in the diagnosis of breast diseases at 3.0T MR, in order to determine the reasonable scan sequence.Methods: A retrospective analysis of MRI examinations was performed in 60 patients with histologically proved breast diseases. Compare the sensitivity and specificity of three groups of methods of scan+DWI and scan+DCE and scan+DWI+DCE for diagnosis of breast diseases.Results: The sensitivity and specific degree of three groups of of Scan+DWI, scan+DCE, scan+DWI+DCE are 67.6%, 89%, 86% and 88%, 84%, 88%.The detecting rate are significant difference(x2=9.271,P<0.05), and there are statistically significant differences. The comparison between groups of Scan+DWI and scan+DCE is remarkable difference(x2=6.773,P<0.05). The comparison between groups of Scan+DWI+DCE and scan+DCE is no remarkable difference, there isn’t statistic meaning in the difference(x2=0.158,P>0.05).Conclusion: Scan+DCE have high value in the diagnosis of breast tumors, which can be used as a standard scan sequence for preoperative breast diseases.
