目的 探讨出院电话随访对糖尿病患者自我管理行为及血糖的影响.方法 将100例患者随机分为观察组和对照组各50例,两组患者出院时均给予常规的出院指导,观察组在此基础上给予饮食知识、用药指导、运动指导、并发症预防等个性化护理干预并电话随访6个月.比较两组患者出院后6个月自我管理行为及血糖情况.结果 观察组患者出院后6个月自我管理行为能力评分明显高于对照组(P<0.05);干预后血糖及糖化血红蛋白较干预前有所下降,观察组比对照组改善显著(P<0.05).结论 出院后定期电话随访,并给予个体化健康教育,有利于提高患者自我管理能力,有效维持血糖在较理想的范围.

Objective To explore the effects of telephone follow-up after discharging on the self-management behavior and the blood concentration of glucose in patients with diabetes mellitus.Methods100 patients were randomly allocated into an observation group and a control group,with 50 patients in each.All of the patients were given the conventional discharge guidance when they discharged from hospital,on the basis of that,the patients in observation group would receive the additional individualized nursing intervention,such as diet knowledge,medication guidance,exercise guidance,prevention of complications and so on.However,all of the patients were followed up on telephone for 6 months.Self-management behavior and blood glucose in 6 months after discharging were compared between the two groups.Results Self - management behavior score of patients in the observation group was significantly higher than that of the patients in the control group (P<0.05)in the 6 months after discharging; After the intervention the concentration of blood glucose and glycosylated hemoglobin significantly decreased than before,the concentration of blood glucose and glycosylated hemoglobin of patients in the observation group decreased more than that of the patients in the control group (P<0.05).Conclusion Regular telephone follow-up and individualized health education after discharging are helpful for improving the patients' ability of self-management,and effective for maintaining the blood concentration of glucose in an ideal range.
