目的：探讨难治性功能性消化不良（FD）患者的工作、运动、睡眠及就医行为特征。方法采用多中心、前瞻性调查设计，对象取自2012年6月至2012年9月广东地区3家三甲医院消化专科门诊就诊，并经罗马Ⅲ标准确诊的1341例FD患者和100例健康志愿者，将受试者分为难治性FD（RFD）、非难治性FD（NRFD）和健康对照组。结果①工作行为：RFD组无工作发生率明显高于健康组（P<0.05），倒班率、2班制发生率明显低于健康组（P<0.05），RFD在轮班方式、轮班制度（除1班制外）方面与NRFD患者差异均无统计学意义（P＞0.05）。②运动行为：三组在体力活动能量消耗方面，差异不显著（P＞0.05），RFD组体力活动水平低的比例最高，且RFD＞NRFD＞健康人（P<0.05）。③睡眠行为：RFD组睡眠障碍发生率最高，且入睡时间延长、实际睡眠时间缩短的现象更为显著（P＜0.05）。④就诊行为：RFD组内镜检查次数、就诊次数、既往耗费均显著高NRFD组。结论 RFD病程长，病情严重，多伴有工作、运动、睡眠及就医行为异常。
목적：탐토난치성공능성소화불량（FD）환자적공작、운동、수면급취의행위특정。방법채용다중심、전첨성조사설계，대상취자2012년6월지2012년9월엄동지구3가삼갑의원소화전과문진취진，병경라마Ⅲ표준학진적1341례FD환자화100례건강지원자，장수시자분위난치성FD（RFD）、비난치성FD（NRFD）화건강대조조。결과①공작행위：RFD조무공작발생솔명현고우건강조（P<0.05），도반솔、2반제발생솔명현저우건강조（P<0.05），RFD재륜반방식、륜반제도（제1반제외）방면여NRFD환자차이균무통계학의의（P＞0.05）。②운동행위：삼조재체력활동능량소모방면，차이불현저（P＞0.05），RFD조체력활동수평저적비례최고，차RFD＞NRFD＞건강인（P<0.05）。③수면행위：RFD조수면장애발생솔최고，차입수시간연장、실제수면시간축단적현상경위현저（P＜0.05）。④취진행위：RFD조내경검사차수、취진차수、기왕모비균현저고NRFD조。결론 RFD병정장，병정엄중，다반유공작、운동、수면급취의행위이상。
Objective To investigate the characteristics of work, exercise, sleep and medical treatment in patients with intractable functional dyspepsia (FD). Methods A multi-center and prospective investigation was conducted which recruited 1,341 consecutive FD outpatients from three hospitals according to the RomeⅢcriteria between June to September in 2012.At the same time, 100 healthy volunteers were enrolled. The subjects were divided into three groups:refractory functional dyspepsia (RFD), non-refractory functional dys-pepsia(NRFD)and healthy controls. Results ①Work behaviors:The rate of RFD in patients with jobless was significantly higher than that of healthy control. The shift rate, 2 shifts were significantly lower than the healthy controls (P<0.05). There was no statistical difference between RFD and NRFD except for one shift. ②Exer-cise behaviors:there was no significant difference of physical activity and energy expenditure among the three groups. Highest rate of low level of physical activity was found in RFD, which indicated that RFD higher than NRFD, and NRFD higher than healthy controls (P<0.05).③Sleep behaviors:The highest incidence of sleep disorders was found in RFD, the time to fall sleep extended, and the real sleep time became shorter than in NRFD and healthy controls (P<0.05).④Health care-seeking behaviors:The frequency of endoscopic exami-nations, care seeking and medical costs were significantly higher in RFD than those in NRFD. Conclusions Patients with RFD had longer disease duration and a more severe form of the disease than patients with NRFD. Moreover, the symptoms were most accompanied with abnormal work, exercise, sleep and health care-seeking behaviors.
