目的：比较他克莫司与环磷酰胺联合糖皮质激素治疗狼疮性肾炎的效果及安全性。方法选取2009年12月至2012年12月本院收治的狼疮性肾炎患者180例为研究对象，随机分为观察组和对照组，两组患者均给予泼尼松治疗，观察组联合他克莫司治疗，对照组间断性给予环磷酰胺冲击治疗，比较两组患者治疗后血清细胞及免疫因子水平、疗效及不良反应发生率。结果观察组患者治疗后24小时尿蛋白定量、补体C3水平、ANA阳性率、系统性红斑狼疮疾病活动指数（SLEDAI）评分及不良反应发生率均明显低于对照组（P＜0.05），治疗总有效率明显高于对照组（P＜0.05）。结论他克莫司联合糖皮质激素可降低狼疮性肾炎患者血清细胞及免疫因子水平，疗效显著，不良反应发生率低，值得临床推广应用。
목적：비교타극막사여배린선알연합당피질격소치료랑창성신염적효과급안전성。방법선취2009년12월지2012년12월본원수치적랑창성신염환자180례위연구대상，수궤분위관찰조화대조조，량조환자균급여발니송치료，관찰조연합타극막사치료，대조조간단성급여배린선알충격치료，비교량조환자치료후혈청세포급면역인자수평、료효급불량반응발생솔。결과관찰조환자치료후24소시뇨단백정량、보체C3수평、ANA양성솔、계통성홍반랑창질병활동지수（SLEDAI）평분급불량반응발생솔균명현저우대조조（P＜0.05），치료총유효솔명현고우대조조（P＜0.05）。결론타극막사연합당피질격소가강저랑창성신염환자혈청세포급면역인자수평，료효현저，불량반응발생솔저，치득림상추엄응용。
ObjectiveTo compare the efficacy and safety of tacrolimus and cyclophosphamide in the treatment of lupus nephritis.MethodFrom December 2009 to December 2012, 180 patients with lupus nephritis were treated as the research objects, they were divided into observation group and control group randomly. Two groups of patients were given prednisone treatment, observation group accepted tacrolimus, control group accepted cyclophosphamide. Compared the two groups of patients after treatment of serum cells and immune factors, efifcacy and adverse reactions.ResultThe 24 hours urine protein, level of C3, ANA positive rate, systemic lupus erythematosus disease activity index (SLEDAI) scores and adverse reactions of observation group were signiifcantly lower than control group, the total effective rate was signiifcantly higher than control group (P<0.05).ConclusionTacrolimus combined with cyclophosphamide can reduce the level of serum cells and immune factors in patients with lupus nephritis, the effect is more signiifcant, the incidence of adverse reactions is low, worthy of clinical application.
