目的研究右美托咪定用于高龄患者行硬膜外麻醉下前列腺电切术后患者认知功能的变化情况.方法选取2013年1月至2014年12月我院接受硬膜外麻醉下行前例腺电切术的高龄患者76例并分为研究组与对照组.对比两组的手术时间、不同时间点MAP与HR以及治疗前后MMSE评分.结果给药前两组患者MAP与HR相差不大,手术中与手术后研究组MAP与HR较对照组相对更低;两组患者在治疗前所得MMSE评分相差不大,但在手术后对照组患者所得MMSE评分有很大程度的下降且得分显著低于研究组,以上差异具有统计学意义(P<0.05).结论右美托咪定可抑制促炎因子的释放及氧自由基的产生,能够有效减少老年患者硬膜外麻醉术后认知功能的影响,值得应用.

Objective To study the influence of dexmedetomidine on postoperative cognitive functionfor elderly patients undergoing transurethral resection of the prostateunderepidural anesthesia.Methods From January 2013 to December 2014,a total of 76 caseselderly patients receivedtransurethral resection of the prostateunderepidural anesthesiain our hospital were enrolled in the study and they were divided into study group and control group.The operation time,HR and MAP of the two groups were compared,and the MMSE score before and after the treatment was compared.Results The MAP and HR had little difference between the two groups before giving drug,intra-operative and post-operative MAP and HR of study group waslower compared with control group.Before the treatment,the differenceof MMSE score between the two groups was not statistically significant.But after the operation the MMSE score in control group was greatly decreased and the score was significantly lower than that in the study group,the differences with statistical significance (P <0.05).Conclusions Dexmedetomidine can reduce the response of inflammatory cell,inhibit Pro-inflammatory cytokine release and oxygen free radical generation,can effectively reduceinfluence on postoperative cognitive functionof the elderly patients with prostate cut under epidural anesthesia,it is worth popularization and application.
