目的 探讨热牙胶垂直加压充填技术在根尖封闭性研究中的临床价值.方法 采集所在三级甲等医院口腔颌面外科,因牙周疾患拔出的新鲜离体单根牙80颗,随机分为A、B两组.A组(40颗)采用Pro-Taper镍钛机扩预备;B组(40颗)采用不锈钢K锉手动预备.将根备后的A组B组样本牙,各自随机分出A1、A2组和B1、B2组,对以上A1、B1组采用热牙胶垂直加压充填技术;对A2、B2组采用传统冷牙胶侧方加压充填技术,在实验中比较两种根充技术对根尖所产生的微渗漏量,进一步分析热牙胶垂直加压充填技术对根尖封闭性的影响.结果 80例样本牙中,采取不同根管预备方式对根尖区产生的微渗漏量并无明显差异(根备总效应P>0.05);而选择两种根管充填技术,其根尖区产生的微渗漏量存在明显差异,具统计学意义(P<0.05).结论 热牙胶垂直加压充填技术的根尖封闭性能优于传统冷牙胶侧方加压充填技术,明显提高了根管治疗的成功率,值得推广.

Objective Study on the clinical value of hot gutta-percha vertical condensation in sealing performance of apical zone.Methods All the samples in this research were collected from oral and maxillofacial surgery clinic of the fourth clinical hospital of Harbin Medical University,the 80 teeth pulled out for orthodontia or periodontal disease.80 human teeth were randomly divided into group A and B.Group A(40 teeth) used Nickel-titanium rotary device to prepare root canal,however,group B used Mani K file to prepare root canal manually.And then Group A was divided to A1 and A2,Group B was divided into B1 and B2.A1 and B1 filled by hot gutta-percha vertical condensation,A2 and B2 filled by cold lateral condensation.The micro leakage of apical zone of different root canal condensation technique were compared,achieving the effect of hot gutta-percha vertical condensation on sealing performance of apical zone.Results For all the 80 samples,there was no obviousdifferenceof micro leakage of two different root canal preparation methods(P>0.05).But the difference of micro leakage between the two root canal condensation techniquewas statistically significant (P <0.05).Conclusions Hot gutta-percha vertical condensation technique is better than cold lateral condensation technique in sealing performance of apical zone,it can improve the cure rate of root canal treatment,worth to be popularized.
