目的 探讨帕瑞昔布术后镇痛对乳腺癌患者术后血浆VEGF-C、TNF-α、IL-1β的影响.方法 拟行乳腺癌改良根治术患者40例,随机分为舒芬太尼组(SF组)和帕瑞昔布组(P组).分别在麻醉诱导前、手术结束时、术后48 h时测定血浆中VEGF-C、TNF-α、IL-1β浓度.记录两组患者术后2、24、48 h VAS、术后48 h内不良反应(恶心、呕吐)的发生例数及舒芬太尼消耗总量.结果 与SF组比较,P组患者术后48 h血浆VEGF-C、TNF-α、IL-1β浓度均显著降低(P<0.05).两组患者不同时点VAS评分差异无统计学意义(P>0.05),但SF组患者不良反应的发生率明显高于P组(P<0.05).术后48 h内舒芬太尼消耗总量SF组多于P组(P<0.05).结论 帕瑞昔布术后镇痛能明显降低乳腺癌患者术后血浆VEGF-C、TNF-α、IL-1β的浓度.

Objective To evaluate the effects ofparecoxib on serum VEGF-C,TNF-αand IL-1β concentrations in female patients undergoing surgery for breast cancer during postoperative analgesia.Methods Forty female patients with primary breast cancer undergoing modified radical mastectomy were randomly divided into receiving postoperative analgesia with parecoxib (P group) and sufentanil (SF group) respectively.Venous blood was sampled before anesthesia, at the end of surgery and at 48 h after the surgery.The samples were analyzed to determine the serum VEGF-C, TNF-α, IL-1 β concentrations.VAS was recorded at postoperative 2,24,48 h respectively.Side effects (including nausea and vomiting) and the amount of sufentanil used during 48 h were also recorded.Results Compared with group SF, the serum levels VEGF-C, TNF-α, IL-1β in group P decreased markedly (P < 0.05).No difference was found in V AS between the two groups (P> 0.05) in all observed time.The incidence of side-effects and the consumption of sufentanil during postoperative 48 h in group P were less than those in group SF (P< 0.05).Conclusion Postoperative analgesia with parecoxib decreases the serum concentrations of VEGF-C, TNF-α, IL-1 β in female patients undergoing surgery for breast cancer.
