目的:探讨鼠双微基因( MDM2)、人第10号染色体缺失的磷酸酶及张力蛋白同源的基因( PTEN)基因多态性及单倍型与安阳地区食管鳞癌易感性的关系。方法提取安阳地区226例食管癌患者及226例健康献血者外周血基因组DNA,采用聚合酶链反应-限制性片段长度多态性法检测MDM2 T309G、PTEN rs701848、PTEN rs2735343位点基因型,采用条件Logistic回归模型分别进行基因多态性、单倍型与食管鳞癌易感性的相关性分析。结果 PTEN rs710848 TC、CC 基因型和PTEN rs2735343 CG、GG基因型均可增加食管癌发病风险(P=0.001,OR=2.29,95% CI 1.42~3.70;P=0.046,OR=1.86,95% CI 1.01~3.41;P=0.001,OR=2.76,95% CI 2.58~4.82;P<0.001,OR=4.30,95%CI 2.27~8.13),而MDM2 T309G基因多态性在两组间的分布无统计学意义(P>0.05)。在PTEN单倍型分析中,--/GC单倍型增加食管癌发病风险( P<0.001),而CT/CT单倍型则降低其风险( P<0.001)。结论 PTEN rs701848与PTEN rs2735343基因多态性与安阳地区食管鳞癌易感性相关,PTEN--/GC单倍型可能增加食管癌发病风险,而CT/CT单倍型则可能降低其发病风险。

목적:탐토서쌍미기인( MDM2)、인제10호염색체결실적린산매급장력단백동원적기인( PTEN)기인다태성급단배형여안양지구식관린암역감성적관계。방법제취안양지구226례식관암환자급226례건강헌혈자외주혈기인조DNA,채용취합매련반응-한제성편단장도다태성법검측MDM2 T309G、PTEN rs701848、PTEN rs2735343위점기인형,채용조건Logistic회귀모형분별진행기인다태성、단배형여식관린암역감성적상관성분석。결과 PTEN rs710848 TC、CC 기인형화PTEN rs2735343 CG、GG기인형균가증가식관암발병풍험(P=0.001,OR=2.29,95% CI 1.42~3.70;P=0.046,OR=1.86,95% CI 1.01~3.41;P=0.001,OR=2.76,95% CI 2.58~4.82;P<0.001,OR=4.30,95%CI 2.27~8.13),이MDM2 T309G기인다태성재량조간적분포무통계학의의(P>0.05)。재PTEN단배형분석중,--/GC단배형증가식관암발병풍험( P<0.001),이CT/CT단배형칙강저기풍험( P<0.001)。결론 PTEN rs701848여PTEN rs2735343기인다태성여안양지구식관린암역감성상관,PTEN--/GC단배형가능증가식관암발병풍험,이CT/CT단배형칙가능강저기발병풍험。
Objective To observe the correlation between the polymorphisms and haplotype of mouse double minute 2(MDM2)and phosphatase and tensin homolog deleted on chromosome 10(PTEN)and susceptibility of esophageal squamous cell carcinoma (ESCC)in Anyang area.Methods Genomic DNA was extracted from peripheral blood leukocytes of 226 patients with ESCC and 226 healthy controls .Genotypes of MDM2 T309G, PTEN rs701848 and rs2735343 were determined by polymerase chain reaction-restriction fragment length polymorphism method ( PCR-RFLP) .Conditional logistic regression was used for correlation analysis.Results Risk of ESCC was elevated in subjects with PTEN rs 701848 TC(P=0.001,OR=2.29, 95%CI:1.42-3.70)and CC(P=0.046,OR=1.86,95% CI:1.01-3.41 )and PTEN rs2735343 CG(P=0.001 ,OR=2.76 , 95% CI:2.58-4.82 ) and GG ( P <0.001 , OR =4.30 , 95% CI:2.27-8.13 ) .The difference of MDM2 T309 Gpolymorphisms distribution between the two groups was not statistically significant ,The haplotype--/GC of PTEN increased the risk of ESCC ( P<0.001 ) , while CT/CT showed a protective effect ( P <0.001 ) .Conclusion PTEN rs701848 and rs2735343 polymorphisms may be susceptibility factors for the development of ESCC in Anyang area .Haplotype--/GC of PTEN may be a critical risk factor of the susceptibility to ESCC ,while CT/CT plays a protective role .
