目的:观察清毒汤对实验性重症肝损伤大鼠丙氨酸氨基转移酶( alanine aminotransferase,ALT)、内毒素、凝血酶原活动度(prothrombin activity,PTA)及 D-乳酸表达的影响,探讨该方治疗慢性重型肝炎内毒素血症的作用机制。方法建立 TAA 致实验性大鼠重症肝损伤模型,清毒汤干预后,检测大鼠 ALT、内毒素、PTA 及 D-乳酸的表达水平。结果与正常组比较,模型组大鼠 ALT、内毒素、D-乳酸水平均显著升高(P<0.01),PTA 水平显著下降(P<0.01);与模型组比较,清毒汤低、中剂量组及乳果糖组血清 ALT、内毒素、PTA 及 D-乳酸表达水平均有不同程度的改善(P<0.05),清毒汤高剂量组除血清内毒素含量外各指标均明显改善(P<0.05);与乳果糖组比较,清毒汤中剂量组除 PTA 外各指标均不同程度降低( P <0.05),低剂量组各指标均无明显差异(P>0.05),高剂量组除 PTA 外各指标明显高于乳果糖组(P<0.05)。结论清毒汤对实验性肝损伤大鼠有较好的改善肝损伤和降低内毒素表达的作用,尤其中剂量组最为明显,推测其作用机制可能与降低肠道通透性相关。

목적:관찰청독탕대실험성중증간손상대서병안산안기전이매( alanine aminotransferase,ALT)、내독소、응혈매원활동도(prothrombin activity,PTA)급 D-유산표체적영향,탐토해방치료만성중형간염내독소혈증적작용궤제。방법건립 TAA 치실험성대서중증간손상모형,청독탕간예후,검측대서 ALT、내독소、PTA 급 D-유산적표체수평。결과여정상조비교,모형조대서 ALT、내독소、D-유산수평균현저승고(P<0.01),PTA 수평현저하강(P<0.01);여모형조비교,청독탕저、중제량조급유과당조혈청 ALT、내독소、PTA 급 D-유산표체수평균유불동정도적개선(P<0.05),청독탕고제량조제혈청내독소함량외각지표균명현개선(P<0.05);여유과당조비교,청독탕중제량조제 PTA 외각지표균불동정도강저( P <0.05),저제량조각지표균무명현차이(P>0.05),고제량조제 PTA 외각지표명현고우유과당조(P<0.05)。결론청독탕대실험성간손상대서유교호적개선간손상화강저내독소표체적작용,우기중제량조최위명현,추측기작용궤제가능여강저장도통투성상관。
Objective To observe the effects of Qingdu decoction on the expressions of alanine aminotransferase (ALT), endotoxin, prothrombin activity (PTA) and D-lactic acid in rats with severe liver injury and explore the mechanism of Qingdu decoction on treating Chronic severe hepatitis complicated with endotoxemia. Methods The severe liver injury rat model was induced by Thioacetamide ( TAA), and treated with Qingdu decoction. And then the blood level of ALT, endotoxin, PTA and D-lactic acid were measured. Results The expression levels of ALT, endotoxin and D-lactic acid in the model group were significantly increased and PTA was significantly decreased compared to that of in the Normal Group (P<0. 01). Compared to the model group, the level of ALT, endotoxin, PTA and D-lactic acid in middle and low dose Qingdu decoction groups and Lactulose group were improved significantly (P<0. 05). All indexes expressions except endotoxin in the high dose Qingdu decoction group were decreased significantly (P<0. 05). Compared to the Lactulose group, all indexes expressions except PTA in the middle dose Qingdu decoction group were decreased significantly (P<0. 05). All indexes in the low dose group had no significant difference (P>0. 05), while all indexes expressions except PTA in the high dose group were significantly higher than the Lactulose group (P<0. 05). Conclusion Qingdu decoction can improve liver injury and decrease endotoxin in rats with experimental hepatic injury complicated with endotoxemia, especially in the middle dose group. The mechanism could be related to the reduction of intestinal permea-bility.
