目的:分析1-36月婴幼儿的宽频声能吸收率(wideband acoustic energy absorbance ,WBA)测试结果,为其临床应用提供参考。方法选取1-36月、声导抗测试结果正常、畸变产物耳声发射,测试正常的婴幼儿共172名(337耳)。根据月龄分组:1-2月组,共86耳;3-6月组,共61耳;7-12月,共97耳;13月-36月组,共93耳。对各组婴幼儿进行宽频声能吸收率测试,比较各个月龄段的声能吸收率曲线分布特点,分析吸收率随年龄增长的变化情况。结果1、声能吸收率随月龄变化,频率-吸收率曲线逐渐由“~”转变成“∧”形。2、7-12月和13-36月组中有1/2的频率的声能吸收率值差别无统计学意义。3、四组声能吸收率各个频率的标准差SD(standard deviation)的均值分别为0.2066、0.1731、0.1393、0.1583。结论1、随月龄增加,频率-吸收率曲线逐渐由“~”转变成“∧”形,中高频的声能吸收率效能逐渐高于低频和高频。2、7月以后,吸收率曲线形态逐渐趋于稳定,组内变化差异减小。3、随月龄的变化声能吸收率曲线范围也逐渐变化,当在容许的变化范围内波动时,仍可视为正常中耳。且月龄越小,声能吸收率值波动越大,表明中耳所处的发育状态越不稳定。

Objective Wideband acoustic energy absorbance (WBA) was tested in 172 infants aged 1-36 months in an attempt to provide references for potential clinical applications. Methods Infants (n=172, 337 ears) aged 1-36 months who had passed tympanometry and distortion product otoacoustic emission (DPOAE) test were divided into 4 groups based on their age: Group 1(1-2 months, 86 ears), Group 2 (2-6 months, 61 ears), Group 3 (6-12 months, 97 ears) and Group 4 (above 12 months, 93 ears). All the infants re-ceived WBA test. Variations of absorbance among different groups were compared and relations between ab-sorbance characteristics and age were investigated. Results 1) Along with age, the WBA frequency-absor-bance curve changed from a“~”shape to a“^”shape. 2) WBA showed no significant differences for half of the tested frequencies between Groups 3 and 4. 3) The average WBA standard deviations for the four age groups were 0.2066, 0.1731, 0.1393 and 0.1583, showing a trend of decrease with age. Conclusions 1) WBA frequency-absorbance rate curves change with age, with gradually increasing mid frequency WBA relative to low and high frequencies. 2) The frequency-absorbance rate curve appears to stabilize after 7 months. 3) WBA variations also decrease along with age, indicating variable middle ear development at younger ages.
