目的：探讨胎盘及脐血中基质金属蛋白酶-1(matrix metalloproteinases,MMP-1)及其抑制因子-1(tissue Inhibitor of metalloproteinases-1,TIMP-1)表达与妊娠期高血压疾病(hypertensive disorder complicating pregnancy, HDCP)发生及疾病严重程度的相关性。方法选择2012年1月至2014年1月湖北医药学院附属太和医院产科收治的86例HDCP患者为研究组,其中妊娠期高血压组36例、轻度子痫前期组18例和重度子痫前期组32例,选取同期正常妊娠晚期孕妇60例为对照组。采用免疫组化( SP)法检测各组胎盘中MMP-1、TIMP-1的表达；酶联免疫吸附法( ELISA)检测脐血血清中MMP-1、TIMP-1的表达。结果胎盘中MMP-1在对照组、妊娠期高血压组、轻度子痫前期组、重度子痫前期组的阳性表达率分别为96．7%、77．8%、66．7%、23．1%；4组脐血血清中MMP-1的表达分别为(294．33±11．53) pg/mL、(247．78±20．32) pg/mL、(177．67±12．63) pg/mL、(124．68±15．41) pg/mL,4组间两两比较差异均有统计学意义(P<0．05)。4组胎盘和脐血血清中TIMP-1的表达差异无统计学意义(P>0．05)。结论 MMP-1与HDCP的发生和疾病严重程度之间存在相关性。
목적：탐토태반급제혈중기질금속단백매-1(matrix metalloproteinases,MMP-1)급기억제인자-1(tissue Inhibitor of metalloproteinases-1,TIMP-1)표체여임신기고혈압질병(hypertensive disorder complicating pregnancy, HDCP)발생급질병엄중정도적상관성。방법선택2012년1월지2014년1월호북의약학원부속태화의원산과수치적86례HDCP환자위연구조,기중임신기고혈압조36례、경도자간전기조18례화중도자간전기조32례,선취동기정상임신만기잉부60례위대조조。채용면역조화( SP)법검측각조태반중MMP-1、TIMP-1적표체；매련면역흡부법( ELISA)검측제혈혈청중MMP-1、TIMP-1적표체。결과태반중MMP-1재대조조、임신기고혈압조、경도자간전기조、중도자간전기조적양성표체솔분별위96．7%、77．8%、66．7%、23．1%；4조제혈혈청중MMP-1적표체분별위(294．33±11．53) pg/mL、(247．78±20．32) pg/mL、(177．67±12．63) pg/mL、(124．68±15．41) pg/mL,4조간량량비교차이균유통계학의의(P<0．05)。4조태반화제혈혈청중TIMP-1적표체차이무통계학의의(P>0．05)。결론 MMP-1여HDCP적발생화질병엄중정도지간존재상관성。
Objective To investigate the relationship between the expression of matrix metallopreteinases-1 (MMP-1), tissue inhibitor of metalloproteinases-1 (TIMP-1) and hypertensive disorder complicating pregnancy (HDCP). Methods The 86 patients with HDCP in Taihe Hospital Affiliated to Hubei Medical College from Jan 2012 to Jan 2014 were enrolled as the study group, including 36 patients with gestational hypertension were as gestational hypertension group, 18 cases with mild preeclampsia were as mild preeclampsia group, and 32 cases with severe preeclampsia were as severe preeclampsia group. 60 normal pregnant women were collected as control group. Immunohistochemistry was used to analyze MMP - 1 and TIMP - 1 in placenta. Enzyme linked immunosorbent assay was used to analyze the expression of MMP-1 and TIMP-1 in umbilicus serum. Results In the placenta, the rate of positive expression of MMP-1 of control group, gestational hypertension group, preeclampsia group, severe preeclampsia group were 96. 7 %, 77. 8 %, 66. 7 % and 23. 1 % respectively. In umbilicus serum, the MMP - 1 of control group gestational, hypertension group, preeclampsia group, severe preeclampsia group were ( 294. 33 ± 11. 53 ) pg/mL, ( 247. 78 ± 20. 32 ) pg/mL, (177. 67 ± 12. 63) pg/mL and (124. 68 ± 15. 41) pg/mL respectively, there were significant differences between groups (P<0. 05). But there was no statistically significant difference between groups in the expression of TIMP-1(P>0. 05). Conclusion There is a close relationship between the expression of MMP-1 and HDCP.
