目的：探讨腹腔镜治疗剖宫产瘢痕妊娠( cesarean scar pregnancy,CSP)术中中转开腹手术的原因。方法回顾性分析2011年7月至2013年12月郑州市妇幼保健院妇产科治疗的196例CSP患者中4例中转开腹手术的临床资料,分析腹腔镜治疗CSP术中中转开腹手术的原因。结果4例均顺利完成开腹手术,中转开腹的原因是术野出血多。术后恢复顺利,均治愈出院。结论手术野出血为CSP术中中转开腹的主要原因,严格掌握手术适应证,提高手术医生的操作水平可防止严重并发症的发生。
목적：탐토복강경치료부궁산반흔임신( cesarean scar pregnancy,CSP)술중중전개복수술적원인。방법회고성분석2011년7월지2013년12월정주시부유보건원부산과치료적196례CSP환자중4례중전개복수술적림상자료,분석복강경치료CSP술중중전개복수술적원인。결과4례균순리완성개복수술,중전개복적원인시술야출혈다。술후회복순리,균치유출원。결론수술야출혈위CSP술중중전개복적주요원인,엄격장악수술괄응증,제고수술의생적조작수평가방지엄중병발증적발생。
Objective To investigate the reasons of conversion to laparotomy during the laparoscopy surgery for the cesarean scar pregnancy ( CSP) . Methods Clinical data of 4 patients underwent conversion to laparotomy during laparoscopy surgery among 196 cases with CSP in Maternal and Child Health Hospital of Zhengzhou from July 2011 to December 2013 were retrospectively analyzed, the reasons of conversion were analyzed. Results All the 4 cases were successfully completed laparotomy, the reason of conversion is surgical field bleeding, the 4 cases were cured after operation. Conclusion The surgical field bleeding is the main reason for CSP from laparoscopy surgery to laparotomy surgery. Strictly surgical indications and improvement of operating level can prevent serious complications.
