目的:通过针刺健康青年左侧劳宫穴,应用经颅多普勒超声仪( TCD)观测5个不同时间点大脑中动脉( MCA)的血流动力学变化,为临床应用该穴提供参考依据。方法:随机选取30名健康青年志愿者,男女各15名,针刺左侧劳宫穴,应用TCD检测并记录双侧大脑中动脉在针刺进针前10 min、进针时(进针得气并以平补平泻手法行针15 s后)、留针时(留针15 min并行平补平泻手法行针15 s后)、出针时(留针30 min并以平补平泻手法行针15 s后)、出针后10 min的血流速度及PI和RI的变化。结果:针刺左侧劳宫穴,在出针时和出针后10 min与针刺前相比左侧MCA的收缩期最大血流速度( Vs)降低,舒张期最大血流速度(Vd)和平均血流速度(Vm)升高,搏动指数(PI)及阻抗指数(RI)降低(P<0.05);其余各参数变化均无统计学意义( P>0.05)。结论:针刺左侧劳宫穴,在出针时和出针后10 min可使同侧MCA的Vs下降,Vd和Vm升高,PI和RI均下降。

목적:통과침자건강청년좌측로궁혈,응용경로다보륵초성의( TCD)관측5개불동시간점대뇌중동맥( MCA)적혈류동역학변화,위림상응용해혈제공삼고의거。방법:수궤선취30명건강청년지원자,남녀각15명,침자좌측로궁혈,응용TCD검측병기록쌍측대뇌중동맥재침자진침전10 min、진침시(진침득기병이평보평사수법행침15 s후)、류침시(류침15 min병행평보평사수법행침15 s후)、출침시(류침30 min병이평보평사수법행침15 s후)、출침후10 min적혈류속도급PI화RI적변화。결과:침자좌측로궁혈,재출침시화출침후10 min여침자전상비좌측MCA적수축기최대혈류속도( Vs)강저,서장기최대혈류속도(Vd)화평균혈류속도(Vm)승고,박동지수(PI)급조항지수(RI)강저(P<0.05);기여각삼수변화균무통계학의의( P>0.05)。결론:침자좌측로궁혈,재출침시화출침후10 min가사동측MCA적Vs하강,Vd화Vm승고,PI화RI균하강。
Objective:In order to provide a basis for the clinical application of the Laogong point,we used tran-scranial doppler sonography( TCD) to investigate the blood flow kinetics of the middle cerebral artery( MCA) by acupuncturing young healthy people at the left side of Laogong point in five different time points.Methods:30 healthy volunteers were selected randomly in the experiment,including 15 men and 15 women.We took the TCD to record the changes of MCA's blood flow velocity, pulsation index ( PI ) and resistance index ( RI ) at those checkpoints:10 minutes before acupuncture, 15 seconds after getting the Qi of acupuncture,15 minutes after ac-upuncture,at the time of the needle removal (30 minutes after acupuncture),and 10 minutes after the needle removal .Results:Compared with the time of 10 minutes before acupuncture the left side MCA's peak velocity of systolic(Vs), pulsation index (PI) and resistance index (Rl)were reduced significantly;the peak velocity of diastolic(Vd) and mean velocity(Vm)were increased significantly at the moment of the needle removal and 10 minutes after the needle removal (P<0.05);other parameters did not change significantly.(P>0.05).Con-clusion:When acupunctured on the left Laogong point,the same side MCA's Vs,PI and RI are reduced signifi-cantly , but the Vd and Vm are increased significantly at the moment of the needle removal and 10 minutes after the needle removal.
