目的：对比不同留针时间针刺治疗腰椎间盘突出症的疗效，探究留针时间与疗效的关系。方法：采用随机对照试验方案，将符合纳入标准的54例患者按随机数表分为3组，各组均采用相同的穴位及针刺手法，分别留针15 min、30 min、45 min，10次为一疗程。观察指标包括：简式McGill疼痛量表、重复改良版Schober、中文版Roland－morris功能障碍量表，并进行疗效评估。结果：治疗前后组内比较，各组疼痛积分均显著减低（P＜0．01）、关节活动度均显著提高（P＜0．05）、功能障碍程度均显著降低（P＜0．01）；治疗后组间比较，上述指标及疗效无显著差异（P＞0．05）。结论：针刺治疗腰椎间盘突出症，在改善疼痛症状及功能活动方面疗效显著，但尚不能说明留针时间影响疗效，而在趋势上，留针30 min组和45 min组均优于留针15 min组，当前结果可能与样本量小有关。
목적：대비불동류침시간침자치료요추간반돌출증적료효，탐구류침시간여료효적관계。방법：채용수궤대조시험방안，장부합납입표준적54례환자안수궤수표분위3조，각조균채용상동적혈위급침자수법，분별류침15 min、30 min、45 min，10차위일료정。관찰지표포괄：간식McGill동통량표、중복개량판Schober、중문판Roland－morris공능장애량표，병진행료효평고。결과：치료전후조내비교，각조동통적분균현저감저（P＜0．01）、관절활동도균현저제고（P＜0．05）、공능장애정도균현저강저（P＜0．01）；치료후조간비교，상술지표급료효무현저차이（P＞0．05）。결론：침자치료요추간반돌출증，재개선동통증상급공능활동방면료효현저，단상불능설명류침시간영향료효，이재추세상，류침30 min조화45 min조균우우류침15 min조，당전결과가능여양본량소유관。
Objective:To compare the clinical effect on lumbar disc herniation( LDH) by different needle-re-taining time and explore the possible relationship between needle-retaining time and clinical effects.Methods:The study adopted randomized controlled experiment scheme.54 cases were randomly divided into three groups according to the inclusion order.The needle retaining time was respectively 15 minutes,30 minutes and 45mi -nutes with the same acupoints and acupuncture manipulation,10 times as a period.The McGill Pain Question-naire-short form,the Modified-Modified Schober( MMS) ,and the Chinese version of the Roland-morris Dis-ability Questionnaire( CRMDQ) were obseved before and after treatment.And the curative effect was estimated after treatment.Results:Before and after treatment in every group,the decline in Pain rating index( PRI) scores and Visual Analogue Scale( VAS) scores were significant ( P<0.01);the increase of joint activity was signifi-cant(P<0.05)and the improvement of impairment was significant(P<0.01).Comparison of the pain scores, the joint activity and the degree of dysfunction after treatment:There was no significant difference among three groups on those three measures(P>0.05).Conc lusion:Acupuncture treatment is an effective therapy for LDH which could alleviate pain and dysfunction syndrome.But it has not been proved that needle retaining time has an effect on clinical effect.To analyze the current trend, the total effect of groups with 30 min and 45 min nee-dle retaining is better than the group with 15 min.The current results may be related to small samples.
