目的：分析腹腔镜直肠癌根治术后近期再手术的原因并总结其防治措施。方法回顾腹腔镜直肠癌根治术后近期再手术9例患者的临床资料，分析再手术原因，总结防治措施。结果再手术9例患者中，吻合口瘘3例、肠梗阻3例、术后出血1例、输尿管损伤1例和下肢深静脉血栓1例。再手术8例患者痊愈，1例因感染性休克死亡。9例中7例为前3年腹腔镜手术患者。结论腹腔镜直肠癌根治术后近期再手术与术者经验及熟练度欠缺有一定关系。术中探查全面、仔细，操作精细，术后观察仔细，是提高腹腔镜直肠癌疗效及减少术后再手术率的关键。
목적：분석복강경직장암근치술후근기재수술적원인병총결기방치조시。방법회고복강경직장암근치술후근기재수술9례환자적림상자료，분석재수술원인，총결방치조시。결과재수술9례환자중，문합구루3례、장경조3례、술후출혈1례、수뇨관손상1례화하지심정맥혈전1례。재수술8례환자전유，1례인감염성휴극사망。9례중7례위전3년복강경수술환자。결론복강경직장암근치술후근기재수술여술자경험급숙련도흠결유일정관계。술중탐사전면、자세，조작정세，술후관찰자세，시제고복강경직장암료효급감소술후재수술솔적관건。
Objective To analyze the reasons of reoperation after laparoscopic radical resection of rectal carcinoma and to summarize preventive measures.Methods Clinical data were collected from of nine patients who underwent reop-eration after laparoscopic radical resection of rectal carcinoma before retrospective analysis.Meanwhile, preventive meas-ures were summarized.Results The patients underwent reoperation due to anastomotic fistula (3 patients), intestinal obstruction (3 patients), postoperative hemorrhage (1 patient), ureteral injury (1 patient) and deep venous thrombosis (1 patient) , in which 8 patients were cured, 1 patient died of infective shock.There were seven patients who received laparoscopic surgery within the recent three years.Conclusion Reoperations after laparoscopic radical resection of rectal carcinoma are related to operators'skill and experience shortage.The number of reoperations may be prevented through active and effective methods.
