目的：探讨右美托咪定预防老年人术后谵妄的临床效果。方法选取我院收治的148例行胸科开胸手术患者为研究对象，随机抽取74例为对照组行生理盐水静脉泵注，另74例为研究组行右美托咪定泵注，比较两组患者的临床治疗效果。结果研究组患者的最小肺泡浓度（MAC）值低于对照组，差异有统计学意义（P＜0.05）；研究组的术后镇痛评分＞4分12例，少于对照组的26例，差异有统计学意义（P＜0.05），研究组术后谵妄2例少于对照组的14例，差异有统计学意义（P＜0.05）。结论对于术后老年患者而言，行右美托咪定，预防了谵妄的发生，降低了MAC值，减轻了术后疼痛，提高了患者的生存质量。
목적：탐토우미탁미정예방노년인술후섬망적림상효과。방법선취아원수치적148례행흉과개흉수술환자위연구대상，수궤추취74례위대조조행생리염수정맥빙주，령74례위연구조행우미탁미정빙주，비교량조환자적림상치료효과。결과연구조환자적최소폐포농도（MAC）치저우대조조，차이유통계학의의（P＜0.05）；연구조적술후진통평분＞4분12례，소우대조조적26례，차이유통계학의의（P＜0.05），연구조술후섬망2례소우대조조적14례，차이유통계학의의（P＜0.05）。결론대우술후노년환자이언，행우미탁미정，예방료섬망적발생，강저료MAC치，감경료술후동통，제고료환자적생존질량。
Objective To investigate the clinical effect of dexmedetomidine for prevention of postoperative delirium in elderly patients.Methods Selected our hospital of 148 cases of chest in patients undergoing thoracic surgery for research object,randomly selected 74 cases for the control group received normal saline for intravenous infusion and other 74 cases as study group underwent dexmedetomidine constant infusion,compared two groups of patients with clinical therapeutic effect. Results The patients in the study group of the minimum alveolar concentration(MAC)value was lower than the control group,and the difference was statistically significant(P<0.05),study group of postoperative pain score>4 minutes and 12 cases were less than that in the control group 26 cases,the difference was statistically significant(P<0.05),study group of postoperative delirium in 2 cases less than 14 patients of the control group,the difference was statistically significant(P<0.05).Conclusion For postoperative elderly patients,line of dexmedetomidine,prevent the occurrence of delirium,reduced the value of the Mac,reduce postoperative pain, improve the quality of life of patients.
