目的：探讨血清甘油三脂(TG)对2型糖尿病患者发生非酒精性脂肪肝(NAFLD)风险的预测价值。方法选取2006至2007年河北唐山开滦集团公司员工中符合2型糖尿病诊断标准的研究对象作为观察队列,最终有2802例研究对象(男2344例,女458例)纳入统计分析,按照 TG 和 hsCRP 水平分为正常组、高 TG 组、高 hsCRP 组、高 TG 及 hsCRP组,于2008至2009年度进行第二次体检,比较不同组2型糖尿病患者 NAFLD 的发病率。用 Logistic 回归分析不同组2型糖尿病患者发生 NAFLD 风险。结果①2年间观察队列中有813例发生 NAFLD,发生 NAFLD 的比例为29．0%。其中正常组、高 TG 组、高 hsCRP 组和高 TG 及 hsCRP 组 NAFLD 的发病率分别为22．3．0%、29．2%、30．5%和49．7%,差异有统计学意义(P <0．01)。②高 TG 组、高 hsCRP 组和高 TG 及 hsCRP 组发生 NAFLD 的风险分别是正常组的1．77、1．39、2．43倍,用多因素 Logistic 回归分析校正基线年龄、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、空腹血糖、血尿酸及BMI 后,三组发生 NAFLD 的风险仍分别是正常组的1．64、1．42、2．29倍。结论2型糖尿病患者基线 TG 对发生 NAFLD风险具有一定的预测价值,基线 TG 较高者发生 NAFLD 的风险增加,基线 TG 及 hsCRP 水平较高者发生 NAFLD 的风险更高。

Objective To investigate the predictive significance of triglyrcerid(TG)level on the incidence of nonalcoholic fatty liver(NAFLD)in type 2 diabetes mellitus population. Methods Type 2 Diabetes Mellitus Population were recruited as pro-spective cohort study from the Kailuan Company in 2006 - 2007. After 2-years follow-up,data of 2802 subjects(2344 males and 458 females) aged 22-88 were collected their data available. According to TG and hscrp levels,people were gouped into normal group,high TG group,high hscrp group. in 2008 - 2009 patients undergo secondary physical examination. We compared the incidence rates of NAFLD in groups with respect to baseline triglyrcerid and serum hsCRPlevels. Logistic regression analysis was used to ana-lyze the prognostic significance of baseline triglyrcerid and serum hsCRPlevels on the incidence of NAFLD in type 2 diabetes mellitus. Logistic regression analysis also showed that the risk of development of NAFLD with high triglyrcerid and high serum hsCRPlevels at baseline were higher than those with nomal triglyrcerid and serum hsCRPlevels (OR are 1. 64、1. 42 and 2. 29 respectively)after adjusting of baseline age,gender,TC,HDL-C,LDL-C,FBG,SUA and BMI. Conclu-sion Baseline TG level had predictive significance for NAFLD in Type 2 Diabetes Mellitus Population. high level sof triglyrcerid and serum hsCRPin type 2 diabetes mellitus population had high risk of NAFLD.
