目的:探讨 SYBR Greens 实时荧光定量 PCR 法在乳腺癌诊断中的应用价值。方法以β-actin 为内对照,采用荧光定量 PCR(FQ-PCR)法检测45例健康女性体检者、91例良性乳腺疾病、193例乳腺癌以及87例其他肿瘤患者外周血中人乳腺球蛋白(human mammaglobin,HMAM)的表达量,分析其在不同临床病理因素情况下基因表达量的差异,并与 RT-PCR 法及其他乳腺癌标志物(CEA、CA125和 TSGF)进行灵敏度和特异性的比较。结果乳腺癌患者 HMAM 阳性率明显高于健康体检者、良性乳腺疾病和其他肿瘤患者(P <0.05);良性乳腺疾病和其他肿瘤患者 HMAM 阳性率高于健康体检者(P <0.05);良性乳腺疾病和其他肿瘤患者 HMAM 阳性率比较,差异无统计学意义(P >0.05)。乳腺癌患者 HMAM 表达与年龄、肿瘤大小、乳腺癌类型及雌激素受体、孕激素受体表达无关(P >0.05),与乳腺癌临床分期、组织学分级、腋窝淋巴结转移及 HER2/neu 表达呈正相关(P <0.05)。HMAM 对乳腺癌诊断的灵敏度高于 TSGF 和 CEA(P <0.05),特异性高于 CA153、TSGF 和 CEA(P <0.05)。SYBR Green FQ-PCR 对乳腺癌检测的灵敏度和特异性均高于 RT-PCR(P <0.05)。结论SYBR Green FQ-PCR法检测 HMAM 灵敏度高、特异性强,可有效辅助乳腺癌的诊断。

목적:탐토 SYBR Greens 실시형광정량 PCR 법재유선암진단중적응용개치。방법이β-actin 위내대조,채용형광정량 PCR(FQ-PCR)법검측45례건강녀성체검자、91례량성유선질병、193례유선암이급87례기타종류환자외주혈중인유선구단백(human mammaglobin,HMAM)적표체량,분석기재불동림상병리인소정황하기인표체량적차이,병여 RT-PCR 법급기타유선암표지물(CEA、CA125화 TSGF)진행령민도화특이성적비교。결과유선암환자 HMAM 양성솔명현고우건강체검자、량성유선질병화기타종류환자(P <0.05);량성유선질병화기타종류환자 HMAM 양성솔고우건강체검자(P <0.05);량성유선질병화기타종류환자 HMAM 양성솔비교,차이무통계학의의(P >0.05)。유선암환자 HMAM 표체여년령、종류대소、유선암류형급자격소수체、잉격소수체표체무관(P >0.05),여유선암림상분기、조직학분급、액와림파결전이급 HER2/neu 표체정정상관(P <0.05)。HMAM 대유선암진단적령민도고우 TSGF 화 CEA(P <0.05),특이성고우 CA153、TSGF 화 CEA(P <0.05)。SYBR Green FQ-PCR 대유선암검측적령민도화특이성균고우 RT-PCR(P <0.05)。결론SYBR Green FQ-PCR법검측 HMAM 령민도고、특이성강,가유효보조유선암적진단。
Objective To study the application of SYBR Green fluorescent quantification polymerase chain reaction(FQ-PCR)to detect HMAM mRNA in circulating tumor cells of breast cancer.Methods β-actin used as internal control,HMAMs in the of clinical samples of 45 healthy women,91 patients with benign breast disease,1 93 patients with breast cancer and 87 other cancer patients were detected by SYBR Green FQ-PCR.The different expressions in different clinical pathological factors were analyzed and compared with RT-PCR,CEA,CA125 and TSGF in terms of sensitivity and specificity.Results The positive expression of HMAM in breast cancer patients was markedly higher than those in healthy individuals,benign breast disease patients and other cancer patients(P <0.05 );The positive expression of HMAM in benign breast disease patients and other cancer patients was higher than those in healthy individuals(P <0.05);There was no statistically significant difference in the positive expression of HMAM between benign breast disease patients and other cancer patients(P >0.05 );No significant association of HMAM positive expression with age ,tumor size,type,estrogen receptor and progesterone receptor were noted (P >0.05),but their expression in breast cancer was positively correlated with clinical stage,histological grade, axillary metastatic lymph node and HER2/neu (P < 0.05 ).The sensitivity of HMAM in the diagnosis of breast cancer was significantly higher than those of TSGF and CEA (P < 0.05 )and the specificity of HMAM was significantly higher than those of CA1 53,TSGF and CEA (P <0.05).The sensitivity and specificity of HMAM by FQ-PCR were higher than those of ordinary RT-PCR(P < 0.05 ).Conclusion SYBR Green FQ-PCR is a highly sensitive and specific method for quantitating HMAM which can give objective evidence to the diagnosis of breast cancer.
