目的:探讨别嘌醇所致严重皮肤不良反应(SCAR)与 HLA-B?5801基因的关系。方法将服用别嘌醇后致 SCAR 的21例患者作为别嘌醇 SCAR 组,30例服用别嘌醇后未出现 SCAR 的患者作为别嘌醇耐受组,另选取53例健康体检者作为正常对照组。对2组患者及正常对照组血液进行 HLA-B?5801基因检测,观察其HLA-B?5801阳性基因例数及频率。结果别嘌醇 SCAR 组、别嘌醇耐受组及正常对照组的 HLA-B?5801阳性基因数分别为19,4和10例。别嘌醇 SCAR 组 HLA-B?5801阳性基因频率显著高于别嘌醇耐受组,2组比较差别有统计学意义[OR=61.75,95% CI (10.23,372.64),P <0.001];别嘌醇 SCAR 组 HLA-B?5801阳性基因频率显著高于正常对照组,2组比较差别有统计学意义[OR =40.85,95% CI (8.16,204.64),P <0.001]。结论福州市汉族人服用别嘌醇所致 SCAR 与 HLA-B?5801基因有关。

Objective To investigate correlation between allopurinol-induced severe cutaneous ad-verse reactions(SCAR)and the HLA-B? 5801 gene polymorphism. Methods The study used twenty-one cases suffering from allopurinol-induced SCAR as allopurinol SCAR group,thirty cases of patients who took allopurinol yet without SCAR as allopurinol tolerance group. Further,the study included fifty-three healthy persons as the control group. This study detected HLA-B? 5801 and calculated its frequency in the blood of the following groups:allopurinol SCAR group with severe adverse derma reaction,allopurinol tolerance group,and normal healthy group. Results There were nineteen cases of HLA-B? 5801 positive in allopurinol SCAR group,four cases in allopurinol tolerance group,and ten positive cases in the normal healthy group. The allopurinol SCAR group had a significant high frequency in HLA-B? 5801 gene poly-morphism than the allopurinol tolerance group,and the difference between them was statistically signifi-cant[OR=61.75,95% CI (10.23,372.64),P <0.001 ];the HLA-B? 5801 positive frequency in the allo-purinol SCAR group with severe adverse derma reaction was also higher than the normal healthy group, and the difference between them had statistical significance [OR = 40.85,95% CI (8.1 6,204.64 ), P <0.001]. Conclusion The frequency of allopurinol-induced SCAR was related to HLA-B? 5801 gene polymorphism in Han people in Fuzhou.
