目的:本实验以Ⅱ型胶原诱导的类风湿关节炎大鼠( CIA)为模型,通过观察电针对RA模型大鼠VEGF表达的影响,探讨电针治疗类风湿性关节炎的作用机理。方法:60只雄性的Wistar大鼠,随机取8只为正常组,其余大鼠采用Ⅱ型胶原和弗氏不完全佐剂复制类风湿性关节炎模型。造模后,从造模成功的大鼠中取24只大鼠随机分为模型组、泼尼松组和电针组,每组8只。造模后第21天开始治疗,正常组和模型组用生理盐水(0.1mL/10g)灌胃,泼尼松组用泼尼松药液(0.1mL/10g)灌胃,电针组用0.30cm ×25mm的针灸针针刺大鼠双侧的“足三里”和“昆仑”穴,强度为1~2mA,频率2/100Hz,每次30 min,每日1次,连续治疗10次。分别在造模前、造模第7天、造模第14天、造模第21天和治疗结束后称取大鼠的体质量,并采用ELISA法和蛋白印迹法( WB )检测血清和膝关节组织内VEGF的含量。结果:造模后的大鼠可见食量减少,体质量下降,足爪红肿,双侧踝关节肿胀,活动受限;与正常组相比,模型组大鼠的体质量增长变缓(P<0.05),电针组和泼尼松组与模型组相比差异不明显(P>0.05);与正常组相比,模型组血清和膝关节内的VEGF含量均升高( P<0.05);与模型组相比,经电针和泼尼松治疗后的大鼠血清和膝关节内的VEGF含量均将低( P<0.05);电针组与泼尼松组相比,差异不明显(P>0.05)。结论:(1)电针可以显著改善CIA大鼠的饮食状况,增加体质量,减轻关节肿胀;(2)电针可以减少VEGF的分泌可能就是其中的机制之一。

목적:본실험이Ⅱ형효원유도적류풍습관절염대서( CIA)위모형,통과관찰전침대RA모형대서VEGF표체적영향,탐토전침치료류풍습성관절염적작용궤리。방법:60지웅성적Wistar대서,수궤취8지위정상조,기여대서채용Ⅱ형효원화불씨불완전좌제복제류풍습성관절염모형。조모후,종조모성공적대서중취24지대서수궤분위모형조、발니송조화전침조,매조8지。조모후제21천개시치료,정상조화모형조용생리염수(0.1mL/10g)관위,발니송조용발니송약액(0.1mL/10g)관위,전침조용0.30cm ×25mm적침구침침자대서쌍측적“족삼리”화“곤륜”혈,강도위1~2mA,빈솔2/100Hz,매차30 min,매일1차,련속치료10차。분별재조모전、조모제7천、조모제14천、조모제21천화치료결속후칭취대서적체질량,병채용ELISA법화단백인적법( WB )검측혈청화슬관절조직내VEGF적함량。결과:조모후적대서가견식량감소,체질량하강,족조홍종,쌍측과관절종창,활동수한;여정상조상비,모형조대서적체질량증장변완(P<0.05),전침조화발니송조여모형조상비차이불명현(P>0.05);여정상조상비,모형조혈청화슬관절내적VEGF함량균승고( P<0.05);여모형조상비,경전침화발니송치료후적대서혈청화슬관절내적VEGF함량균장저( P<0.05);전침조여발니송조상비,차이불명현(P>0.05)。결론:(1)전침가이현저개선CIA대서적음식상황,증가체질량,감경관절종창;(2)전침가이감소VEGF적분비가능취시기중적궤제지일。
Objective:Autoimmune collagen type II-induced arthritis ( CIA ) rat models were used to observe the influ-ence of electro-acupuncture(EA) on VEGF so as to study its underlying mechanism in improving RA .Methods:There were 60 male Wistar rats and randomly choose 8 as normal control group , and the rest were used by infreund ’ s adjuvant and type Ⅱ collagen to induce arthritis .24 rats which were successfully induced were randomly divided into three groups:model group, prednisolone group and electro -acupuncture(EA) group,with 8 in eaeh group.Treatment from the 21st day ,the normal group and model group were given saline solution (0.1mL/10g) to fill the stomach, and pred-nisone group was given prednisone solution (0.1mL/10g) to fill the stomach.The EA(2/100Hz,1~2mA) group was applied to “Zusanli”(ST36) and “Kunlun”(BL60) for 30min, once daily for 10 times.Measure the rats'body weight before and after the establishment , and at the end of the treatment .Then determine the plasma VEGF contents in serum and the knee joint cavity by ELISA and Western blot .Results:After modeling ,paw swelling ,ankle swelling ,food volume and weight had different degrees of reduction;in comparison with rats in the control group , the rats in model group gained weight slowly (P<0.05);compared with model group , the prednisolone group and electro -acupuncture group had no significant difference ( P>0.05);in comparison with the normal group , serum VEGF levels within the knee were increased ( P<0 .05 ); in comparison with the model group , the serum VEGF levels within the knee joint increased markedly after treatment ( P<0 .05 ) , while the prednisolone group and electro -acupuncture group had no obvious difference(P>0.05).Conclusion:(1)EA can improve the CIA rats'diet and joint swelling degree significantly , then has an obviously therapeutic effect on CIA rats;(2)The decreased expression of VEGF may be one of the mechanisms .
