目的：分析儿童急性呼吸道感染(ARI)者肺炎支原体(MP)及嗜肺军团菌血清1型(LP1)感染情况及流行病学特点。方法选择2013年1~12月,在四川省妇幼保健院儿科临床诊断为 ARI 的1167例患儿为研究对象。采用间接免疫荧光法检测其血清 MP 及 LP1 IgM 抗体,统计学比较不同季度、年龄及性别的 MP、LP1及MP+LP1混合感染阳性检出率差异。结果1167例 ARI 患儿中,①MP、LP1及 MP+LP1混合感染的阳性检出率分别为25.0%、7.4%及4.3%。②2013年第二季度 MP 阳性检出率(36.5%)显著高于第一、三、四季度(21.2%、23.1%、22.6%),且差异均有统计学意义(χ2=7.57,12.07,13.55；P <0.01)；而 LP1及 MP+ LP1混合感染的阳性检出率在不同季度比较,差异均无统计学意义(χ2=6.57,5.88；P >0.05)。③不同年龄患儿的 MP、LP1及 MP+LP1阳性检出率比较,差异均有统计学意义(χ2=199.01,35.18,19.89；P <0.01),而且随患儿年龄增长,MP、LP1及 MP+LP1阳性检出率逐渐升高。④女性患儿 MP 阳性检出率显著高于男性(29.0% vs 22.6%,χ2=5.88,P <0.05),而不同性别患儿的 LP1及MP+LP1阳性检出率比较,差异则无统计学意义(P >0.05)。结论ARI 患儿有较高 MP 及 LP1阳性检出率,应引起儿科医师高度重视。
목적：분석인동급성호흡도감염(ARI)자폐염지원체(MP)급기폐군단균혈청1형(LP1)감염정황급류행병학특점。방법선택2013년1~12월,재사천성부유보건원인과림상진단위 ARI 적1167례환인위연구대상。채용간접면역형광법검측기혈청 MP 급 LP1 IgM 항체,통계학비교불동계도、년령급성별적 MP、LP1급MP+LP1혼합감염양성검출솔차이。결과1167례 ARI 환인중,①MP、LP1급 MP+LP1혼합감염적양성검출솔분별위25.0%、7.4%급4.3%。②2013년제이계도 MP 양성검출솔(36.5%)현저고우제일、삼、사계도(21.2%、23.1%、22.6%),차차이균유통계학의의(χ2=7.57,12.07,13.55；P <0.01)；이 LP1급 MP+ LP1혼합감염적양성검출솔재불동계도비교,차이균무통계학의의(χ2=6.57,5.88；P >0.05)。③불동년령환인적 MP、LP1급 MP+LP1양성검출솔비교,차이균유통계학의의(χ2=199.01,35.18,19.89；P <0.01),이차수환인년령증장,MP、LP1급 MP+LP1양성검출솔축점승고。④녀성환인 MP 양성검출솔현저고우남성(29.0% vs 22.6%,χ2=5.88,P <0.05),이불동성별환인적 LP1급MP+LP1양성검출솔비교,차이칙무통계학의의(P >0.05)。결론ARI 환인유교고 MP 급 LP1양성검출솔,응인기인과의사고도중시。
Objective To investigate Mycoplasma pneumoniae (MP)and Legionella pneumophila serogroup 1(LP1)infection and epidemiologic characteristics in children with acute respiratory tract infection (ARI).Methods A total of 1 1 67 children who were diagnosed as ARI in Sichuan Provincial Hospital of Women and Children from January to December in 2013 were chosen as study subjects.Indirect immunofluorescence assay was used to detect serum IgM antibody of MP and LP1.The positive detection rates were compared statistically amomg different quarters of 2013,age and gender.Results ①The positive detection rates of MP,LP1 and MP+LP1 were 25.0%,7.4% and 4.3% respectively in 1 1 67 ARI patients.②The positive detection rate of MP in the second quarter of 2013(36.5%)was higher than that in the first, the third and the fourth quarter of 2013 (21.2%,23.1%,22.6%),and the differences were statistically significant(χ2 =7.57,12.07,13.55;P <0.01).There were no significant differences among LP1 and MP+LP1 positive detection rates in different quarters of 2013 (χ2 = 6.57,5.88;P > 0.05 ).③ There were no significant differences among the positive detection rates of MP,LP1 and MP + LP1 in different age of children with ARI (χ2 =1 99.01,35.18,1 9.89;P <0.01),and the positive detection rates of MP,LP1 and MP+LP1 increased with increasing age of children with ARI.④The positive detection rate of MP in female children with ARI was higher than that in male,and the difference was statistically significant(29.0% vs 22.6%,χ2 =5.88,P <0.05);but there were no significant differences of LP 1 and MP+LP1 positive detection rates in different gender (P > 0.05 ).Conclusion The children with ARI had high positive detection rates of MP and LP1,and should be paid enough attention by paediatricians.
