目的 探讨2型糖尿病患者血清中脂肪特异性丝氨酸蛋白酶抑制剂( Vaspin)的水平及其与糖尿病肾病的关系. 方法 随机选取50 例健康对照者与96 例2 型糖尿病患者,根据24 小时尿蛋白排泄率( UAER)分为DM组( UAER<20μg/min) ,微量白蛋白尿组( DN1组,UAER 20~200μg/min﹞和大量白蛋白尿组( DN2组,UAER>200μg/min). 应用ELISA法测定血清Vaspin水平,同时测定空腹血糖( FBG)、空腹胰岛素(FINS)、血脂、糖化血红蛋白(HbA1c)、尿素氮(BUN)、血肌酐(Cr),采用稳态模型-胰岛素抵抗公式( HOMA-IR)评估胰岛素抵抗状态. 结果 血清Vaspin水平依次为对照组<DN2组<DN1组<DM组,差异有显著性(P<0. 05). DM组Vaspin水平与性别、甘油三酯(TG)、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、空腹胰岛素(InFINS)、胰岛素抵抗指数(InHOMA-IR)、高密度脂蛋白胆固醇(HDL-c)显著相关(r值分别为0. 561,0. 239,0. 521,0. 593,0. 320,0. 225,-0. 357,P<0. 05). DN组Vaspin水平与FBG、InFINS、InHOMA-IR、性别显著相关(r值-0. 532,-0. 276,-0. 239, 0. 537,P<0. 05). Logistic回归分析显示:Vaspin、HbA1c、 BMI、InHOMA-IR为糖尿病肾病的危险因素. 结论 Vaspin在糖尿病肾病的发生中起重要作用.

목적 탐토2형당뇨병환자혈청중지방특이성사안산단백매억제제( Vaspin)적수평급기여당뇨병신병적관계. 방법 수궤선취50 례건강대조자여96 례2 형당뇨병환자,근거24 소시뇨단백배설솔( UAER)분위DM조( UAER<20μg/min) ,미량백단백뇨조( DN1조,UAER 20~200μg/min﹞화대량백단백뇨조( DN2조,UAER>200μg/min). 응용ELISA법측정혈청Vaspin수평,동시측정공복혈당( FBG)、공복이도소(FINS)、혈지、당화혈홍단백(HbA1c)、뇨소담(BUN)、혈기항(Cr),채용은태모형-이도소저항공식( HOMA-IR)평고이도소저항상태. 결과 혈청Vaspin수평의차위대조조<DN2조<DN1조<DM조,차이유현저성(P<0. 05). Objective To explore the association between serum visceral adipose tissue derived serine protease inhib-itor ( vaspin) and diabetic nephropathy. Method 94 type 2 diabetic patients and 50 controls were involved in this study. According to progression of nephropathy, patients with diabetes were divided into three groups:T2DM group, microalbuminura group (DN1 group,UAER20 ~200μg/min) and macroalbuminura group (DN2group,UAER>200μg/min) . Human body and biochemical parameters were measured. Serum vaspin were measured by ELISA. Result The levels of serum vaspin were significantly difference between four groups ( NC group<DN2 group<DN1 group<DM group, P<0. 05). In the DM group, the level of vaspin was positively correlated with gender,TG,FBG, HbA1c,InFINS,InHOMA-IR(r=0. 561,0. 239,0. 521,0. 593,0. 320,0. 225,P<0. 05), and negatively correlated with HDL-C(r=-0. 357,P<0. 05). In the DN group,vaspin was negatively correlated with FBG、InFINS、InHOMA-IR(r=-0. 532,-0. 276,-0. 239, P<0. 05),and positively with gender(r=0. 537,P<0. 05). Logistic regression a-nalysis showed that HbA1c,BMI, InHOMA-IR and serum vaspin were influential factors of diabetic nephropathy. Conclusion Vaspin plays a very important role in diabetic nephropathy.
