目的 探讨肿瘤预切除术在拒绝穿刺活检的 T1 期乳腺癌患者保乳手术中的应用价值.方法 将100例有保乳意愿并拒绝穿刺活检的T1期乳腺癌患者随机分为两组各50例,观察组肿瘤活检方式采用肿瘤预切除术,对照组采用常规肿物切除术,对两组患者的手术指标、乳房美学效果等进行对比分析. 结果 观察组在手术时间、术中出血量、一次性手术成功率以及手术并发症发生率上均优于对照组(P<0. 05). 观察组与治疗组的局部复发率及乳房美学效果相近(P>0. 05). 采用1. 0cm切缘宽度,总体切缘阳性率为5%;采用2. 0cm切缘宽度,总体切缘阳性率为0%,两组比较差异无显著性(P>0. 05).结论 对拒绝穿刺活检的T1期乳腺癌患者实施保乳手术时,应用切缘宽度为1. 0cm的肿瘤预切除术是一种创伤小、简便实用、效果满意的方法.

Objective To explore the application value of pre-resection of tumor in breast conserving operation for stage T1 breast cancer who refuse to puncture biopsy. Method 100 cases of stage T1 breast cancer patients who re-fuse to puncture biopsy were selected, they were randomly divided into the observation group(50 cases) and control group(50 cases), pre-resection of tumor was used in the observation group, the control group used conventional tumor resection. The operation index and breast aesthetic effects of both groups were compared and analyzed. Re-sult The observation group in the operation time, amount of bleeding during the operation, the one-time success rate of operation and operation complication rate is superior to the control group(P<0. 05). The local recurrence rate and the effects on aesthetics of breast in the two groups are similar(P>0. 05). Using the 1. 0cm resection margin width, margin positive rate was 5%(5/100);when using the 2. 0cm resection margin width, margin positive rate was 0%(100/100), the difference between the two groups had no statistical significance(P>0. 05). Conclusion In breast conserving operation for stage T1 breast cancer patients who refuse to puncture biopsy, using the pre-resection of tumor with 1. 0cm resection margin width is a method of small trauma, simple and practical, with satisfactory re-sults.
