目的:分析中药饮片调剂中审方的作用,探讨和总结我院审方存在的问题并提出相应的改进策略.方法:对我院2011年2月-2014年4月开具的中药饮片的调剂处方进行回顾性分析,审方改进前17000张处方,审方改进后21000张处方,对比分析审方改进前后调剂的差错率及出错项目.结果:审方改进前出现205次调剂错误,差错率为12.1‰,出错项目主要为配伍禁忌、剂量错误、溶媒选择不当等,而审方改进后仅出现12次调剂错误,差错率为0.565‰,明显低于对照组(P<0.05).结论:中药饮片调剂中的审方是中药饮片调剂工作的重要环节,直接影响患者的用药安全及疗效,正确规范的审方机制能有效减少中药饮片调剂的差错,提高中药调剂的质量.

Objective: To analyze TCM prescription, and discuss the improving strategies. Methods: From February 2011 to April 2014, TCM prescriptions were analyzed, including the 17000 prescriptions (before improvement) and 21000 prescriptions (after improvement). The erroneous rate and erroneous items were analyzed and compared. Results: 205 prescriptions were erroneous before improvement; the erroneous rate was 12.1‰; and the erroneous projects were incompatibility, wrong dose, and inappropriate choice of solvent. After improvement only 12 prescriptions were erroneous; the erroneous rate was 0.565‰, significantly lower than that in the control group (P<0.05). Conclusion: The prescription is an important part in adjusting of TCM medicine, which directly affects the safety and efficacy. The correct mechanism can effectively reduce the errors of TCM medicine, and improve the quality of TCM medicine.
