目的：探讨非体外循环冠状动脉旁路移植术（ OPCABG）对冠状动脉粥样硬化性心脏病（冠心病）合并糖尿病患者的治疗效果。方法将许昌市中心医院2008年1月至2012年12月收治的182例冠心病合并糖尿病患者依据手术方式的不同分为OPCABG组（98例）和体外循环冠状动脉旁路移植术（ CCABG）组（84例），观察两组患者围术期临床资料及预后情况。结果与 CCABG 组比较， OPCABG组呼吸机支持时间、住ICU时间、住院时间、术后输血量、神经系统并发症发生率均显著减少[（8.5±2.2） h比（14.8±6.3） h，（2.4±0.5） d比（3.1±0.7） d，（13.1±3.8） d比（17.4±3.2） d，（448±29） mL比（727±18） mL，2.0％（2／98）比8.3％（7／84）]，差异均有统计学意义（P＜0.05）；两组患者出院后随访半年无死亡及严重并发症发生。结论 OPCABG 治疗冠心病合并糖尿病患者安全可靠。

Objective To discuss the therapeutic effect of off-pump coronary artery bypass grafting (OPCABG) on patients with coronary heart disease complicated with diabetes mellitus.Methods A total of 182 cases of coronary heart disease complicated with diabetes mellitus patients admitted to Xuchang City Central Hospital from Jan.2008 to Dec.2012 were included in the study,according to the different operation modes they were divided into OPCABG group(98 cases) and coronary artery bypass grafting (CCABG) group(84 cases).The perioperative clinical data and prognosis of the two groups were observed.Results Ventilation support time,ICU stay days,days of hospitalization, postoperative blood transfusion,nervous sys-tem complication rate in OPCABG group were significantly decreased compared with CCABG group[(8.5 ± 2.2) h vs(14.8 ±6.3) h,(2.4 ±0.5) d vs (3.1 ±0.7) d,(13.1 ±3.8) d vs (17.4 ±3.2) d,(448 ± 29) mL vs(727 ±18) mL,2.0%(2/98) vs 8.3%(7/84),P <0.05].Two groups of patients were fol-lowed up for six months after discharging,no deaths occurred during the follow-up period,no serious compli-cations occurred.Conclusion OPCABG is a safe and reliable operation for coronary heart disease compli-cated with diabetes mellitus.
