目的:检测自身免疫性疾病患者外周血CD4+CD2+5调节性T细胞、T淋巴细胞亚群、免疫球蛋白、补体、炎性因子及黏附分子的变化,探讨上述指标在疾病发生发展中的作用。方法选取2012年1月至2015年1月收治的150例自身免疫性疾病患者,根据疾病活动情况分为活动期组80例,稳定期组70例,另选取同期进行健康体检的正常人150例为对照组。采用流式细胞术测定3组外周血CD4+CD2+5调节性T细胞、T淋巴细胞亚群( CD3+、CD4+、CD8+)及NK细胞水平。采用免疫透射比浊法测定免疫球蛋白( IgA、IgG、IgM)及补体( C3、C4)水平。采用酶联免疫吸附试验测定炎性因子[肿瘤坏死因子-α(TNF-α)、白介素-10(IL-10)、白介素-17(IL-17)、γ干扰素(IFN-γ)]及黏附分子[细胞黏附分子-1(ICAM-1)、血管内皮细胞黏附分子-1(VCAM-1)、E-选择素(E-selectin)]水平。结果活动期组与稳定期组、对照组比较,CD4+CD2+5调节性T细胞、CD4+、CD4+/CD8+、NK细胞、C3、C4明显降低,CD8+、IgA、IgG、IgM、TNF-α、IL-10、IL-17、IFN-γ、ICAM-1、VCAM-1、E-selectin明显升高,差异有统计学意义( P <0?.05)。结论自身免疫性疾病患者存在炎性反应及免疫功能紊乱,对免疫相关指标及炎性因子、黏附分子的检测有助于评估疾病活动期患者的免疫状态,为判断疾病进展及临床治疗提供实验依据。

목적:검측자신면역성질병환자외주혈CD4+CD2+5조절성T세포、T림파세포아군、면역구단백、보체、염성인자급점부분자적변화,탐토상술지표재질병발생발전중적작용。방법선취2012년1월지2015년1월수치적150례자신면역성질병환자,근거질병활동정황분위활동기조80례,은정기조70례,령선취동기진행건강체검적정상인150례위대조조。채용류식세포술측정3조외주혈CD4+CD2+5조절성T세포、T림파세포아군( CD3+、CD4+、CD8+)급NK세포수평。채용면역투사비탁법측정면역구단백( IgA、IgG、IgM)급보체( C3、C4)수평。채용매련면역흡부시험측정염성인자[종류배사인자-α(TNF-α)、백개소-10(IL-10)、백개소-17(IL-17)、γ간우소(IFN-γ)]급점부분자[세포점부분자-1(ICAM-1)、혈관내피세포점부분자-1(VCAM-1)、E-선택소(E-selectin)]수평。결과활동기조여은정기조、대조조비교,CD4+CD2+5조절성T세포、CD4+、CD4+/CD8+、NK세포、C3、C4명현강저,CD8+、IgA、IgG、IgM、TNF-α、IL-10、IL-17、IFN-γ、ICAM-1、VCAM-1、E-selectin명현승고,차이유통계학의의( P <0?.05)。결론자신면역성질병환자존재염성반응급면역공능문란,대면역상관지표급염성인자、점부분자적검측유조우평고질병활동기환자적면역상태,위판단질병진전급림상치료제공실험의거。
Objective To observe the changes of CD4+CD2+5 FoxP3+regulatory T cells ( Treg) , T lymphocyte cells subsets, immunoglobulin, complements, inflammatory cytokines, complements and adhesion molecules in patients with autoimmune diseases in order to explore their roles in pathogenesis and development of diseases.Methods A total of 150 patients with autoimmune diseases who were admitted into our hospital from January 2012 to January 2015 were enrolled in the study.These patients were divided into active period group ( n =80) and stable period group ( n =70) according to the active status of diseases,besides,150 healthy subjects were served as control group.The CD4+ CD2+5 FoxP3 +Treg, T lymphocyte cells subsets ( CD3+,CD4+, CD8+) and NK cells were detected by flow cytometry.Immunoglobulins ( IgA, IgG, IgM) and complements (C3, C4) were measured by turbidimetric method with automatic biochemical analyzer.Inflammatory cytokines (TNF-α, IL-10, IL-17, IFN-γ) and adhesion molecules (ICAM-1,VCAM-1,E-selectin) were detected by ELISA. Results As compared with those in control group and stable period group, CD4+CD2+5 FoxP3+Treg, CD4+, CD4+/CD8+, NK cells, C3 and C4 in active period group were significantly decreased,however, CD8+, IgA, IgG, IgM, TNF-α, IL-10, IL-17, IFN-γ, ICAM-1, VCAM-1 and E-selectin in active period group were significantly increased ( P <0.05) .Conclusion There exists inflammatory reaction and immunological function disorder in patients with autoimmune diseases,therefor the detection of immune indexes, inflammatory cytokines and adhesion molecules is helpfor to assess the patient’s immune state of and disease’s progress so as to provide experimental basis for clinical treatment.
