目的:研究阿托伐他汀对急性心肌梗死后梗死边缘区和非梗死区心肌细胞瞬间外向钾电流( Ito)的影响。方法30只大耳白兔随机分为正常对照组( CON组)、心肌梗死组(AMI 组)和阿托伐他汀组,每组10只。建立AMI动物模型后,口服阿托伐他汀7 d后,用酶解法分离单个心室肌细胞,采用全细胞膜片钳记录技术分别记录左心室梗死边缘区(AMI-R)和非梗死区(AMI-N)心外膜心肌细胞(Epicardium, Epi) Ito密度。结果 CON组、AMI组细胞电流密度于+70 mV电压时达峰值,AMI组较CON组Epi的I-V曲线整体呈下降趋势,CON组于+70 mV电压时Ito电流密度明显高于AMI组中梗死边缘区和非梗死区( P <0 A.01)。 AMI组中梗死边缘区和非梗死区心外膜电流密度间差异有统计学意义( P <0.01)。 CON组、AMI组梗死边缘区和非梗死区的心外膜细胞的失活曲线趋势之间无明显变化,CON组与AMI组非梗死区半数失活电压比较有统计学意义( P <0.05)。结论 AMI梗死边缘区和非梗死区Epi细胞Ito密度与正常心肌细胞想比存在差异性;阿托伐他汀缓解心肌梗死后左心室梗死边缘区和非梗死区Ito密度的电生理异常有部分改善作用。

목적:연구아탁벌타정대급성심기경사후경사변연구화비경사구심기세포순간외향갑전류( Ito)적영향。방법30지대이백토수궤분위정상대조조( CON조)、심기경사조(AMI 조)화아탁벌타정조,매조10지。건립AMI동물모형후,구복아탁벌타정7 d후,용매해법분리단개심실기세포,채용전세포막편겸기록기술분별기록좌심실경사변연구(AMI-R)화비경사구(AMI-N)심외막심기세포(Epicardium, Epi) Ito밀도。결과 CON조、AMI조세포전류밀도우+70 mV전압시체봉치,AMI조교CON조Epi적I-V곡선정체정하강추세,CON조우+70 mV전압시Ito전류밀도명현고우AMI조중경사변연구화비경사구( P <0 A.01)。 AMI조중경사변연구화비경사구심외막전류밀도간차이유통계학의의( P <0.01)。 CON조、AMI조경사변연구화비경사구적심외막세포적실활곡선추세지간무명현변화,CON조여AMI조비경사구반수실활전압비교유통계학의의( P <0.05)。결론 AMI경사변연구화비경사구Epi세포Ito밀도여정상심기세포상비존재차이성;아탁벌타정완해심기경사후좌심실경사변연구화비경사구Ito밀도적전생리이상유부분개선작용。
Objective To investigate the change of transient outward current ( Ito ) of cardiocytes in infarcted marginal zone (AMI-R) and noninfarcted zone (AMI-N) after acute myocardial infarction in rabbits (AMI),and to observe the effects of atorvastatin on Ito.Methods Thirty female big ear white rabbits were randomly divided into 3 groups:control group,AMI group and atorvastatin group,with 10 rabbits in each group.After AMI animal models were established, the rabbits in atorvastatin group were given atorvastatin orally for 7 days (5mg· kg-1· d-1),then single ventricular myocardium cell was separated by enzymolysis method,finally, the Ito density of epicardium ( Epi) in AMI-R and AMI-N was recorded by whole cells patch clamp technology.Results The Ito density of epicardium (Epi) cells reached peak at +70mV voltage in control group and AMI group.There was a significant difference in the electric current density of AMI group between AMI-R and AMI-N ( P <0.01).There was no significant difference in inactivation curve trend between AMI-R and AMI-N in control group and AMI group,however, there was a significant difference in half inactivation voltage in AMI-R between control group and AMI group ( P <0.05).Conclusion There exists variability in Ito density of Epi cells in AMI-R and AMI-N,as compared with that in normal myocardial cells,and atorvastatin can improve the abnormal Ito density in AMI-R and AMI-N after AMI.
