目的：了解肇庆市第二人民医院（以下简称“我院”）药品不良反应（adverse drug reaction，ADR）报告中的不合理用药情况，促进临床合理用药。方法：收集我院2010—2014年470例ADR报告，对其不合理用药情况进行审核与分析。结果与结论：470例ADR报告中，共有79例不合理用药情况，以儿童及老年患者居多。涉及药物包括抗菌药物、中药及中成药制剂、神经系统用药、调节电解质及酸碱平衡药、心血管系统用药、呼吸系统用药等6大类，其中以抗菌药物为主，其次是中药及中成药制剂。不合理用药以超剂量用药、配伍不当、无适应证用药等较为突出。应引起医药人员的高度重视，加强临床合理用药培训和指导，促进合理用药，避免不必要的ADR发生。
목적：료해조경시제이인민의원（이하간칭“아원”）약품불량반응（adverse drug reaction，ADR）보고중적불합리용약정황，촉진림상합리용약。방법：수집아원2010—2014년470례ADR보고，대기불합리용약정황진행심핵여분석。결과여결론：470례ADR보고중，공유79례불합리용약정황，이인동급노년환자거다。섭급약물포괄항균약물、중약급중성약제제、신경계통용약、조절전해질급산감평형약、심혈관계통용약、호흡계통용약등6대류，기중이항균약물위주，기차시중약급중성약제제。불합리용약이초제량용약、배오불당、무괄응증용약등교위돌출。응인기의약인원적고도중시，가강림상합리용약배훈화지도，촉진합리용약，피면불필요적ADR발생。
OBJECTIVE:To investigate the irrational drug use in the adverse drug reactions ( ADR) reports from the Second People's Hospital of Zhaoqing ( hereinafter referred to as “our hospital”) ,and to promote the rational drug use in clinic .METHODS: 470 cases of ADR reports in our hospital during 2010-2014 were collected , of which the irrational drug use would be reviewed and analyzed .RESULTS & CONCLUSIONS: Of the 470 cases, there were 79 cases of irrational drug use , the majority were children and elderly people .The involved drugs mainly included 6 types:antibiotics , TCM and Chinese patent medicine preparation , drugs of nervous system , drugs that regulate the electrolyte and acid-base imbalance , drugs for cardiovascular diseases and drugs for respiratory system; among which the majority were antibiotics , followed by Chinese patent medicine preparation . The mainly irrational drug use situations were overdose drug use , improper compatibility and no indications for drug use , etc; the medical workers should pay high attention to these phenomenon .It is necessary to strengthen the train and guidance of rational drug use for clinician and pharmacists , so as to promote the ration drug use and avoid the occurrence of unnecessary ADR .
