目的总结探讨产褥期下肢深静脉血栓的发病特点、诊治过程、临床转归,以进一步提高临床诊治水平.方法收集统计2010年10月至2014年6月在本院血管外科及产科住院治疗的产褥期并发下肢深静脉血栓的病例并回顾性分析病例资料.结果共统计目标病例80例,中心型及混合型65例(81.25％).中心型及混合型高位血栓患者常规行滤器及植入导管溶栓治疗,效果满意,个别病例为腔及髂静脉发育畸形,效果不佳.结论产褥期血栓患者均为青年女性,个人预期高.一旦罹患产褥期DVT将严重影响患者生活,因此加强产褥期宣教、预防DVT的发生尤其重要.对于已经发生DVT产妇应做到早发现、早诊断、早抗凝溶栓,尽可能采用积极导管接触式溶栓及预防肺栓塞的发生.

Objective To explore the characteristics of the disease,treatment outcome,and clinical process of lower extremity deep venous thrombosis (LEDVT) in puerperium.Method We collected data of postpartum patients with LEDVT in department of vascular surgery or department of obstetrics of our hospital from October 2010 to June 2014,and carried out retrospective analysis.Results There were 65 cases of central type and mixed type in all of the 80 postpartum patients with LEDVT.Most patients with high location of thrombosis generally underwent filter implantation and thrombolytic therapy through the catheter,with satisfactory effect.But some patients with cavity or iliac venous malformation had poor effect.Conclusions Postpartum patients with LEDVT are young women,and they have high personal expectations.Puerperal LEDVT will seriously affect the life of patients,so strengthening puerperal education,preventing DVT are especially important.For postpartum patients with LEDVT,we should accomplish early discovery,early diagnosis,early anti-coagulation and thrombolytic,use active catheter contact thrombolysis and prevent pulmonary embolism as far as possible.
