目的针对股骨近端方旋刀片髓内钉（PENA）治疗老年患者股骨粗隆间骨折的临床疗效进行研究。方法选取2009年2月~2013年10月在我院采用PENA治疗老年股骨粗隆间骨折的患者36例，骨折按AO分类标准，其中A1型14例，A2型16例，A3型6例。最后进行回顾分析，主要分析其手术时间、术中失血量、骨折愈合情况以及髋关节功能恢复情况。结果经过PFNA治疗后，36例患者的股骨粗隆间骨折均已达到临床愈合，并且术后均未出现感染、内固定失败，也未出现股骨头缺血坏死及髋内翻畸形等症状。并且，评价髋关节功能的总优良率达到了97.9%。结论老年患者股骨粗隆骨折采用PFNA治疗不但创伤小、并且固定牢稳、还可以减少并发症的发生。适合老年患者与对手术耐受力较小的患者，并将其作为首选治疗方案。
목적침대고골근단방선도편수내정（PENA）치료노년환자고골조륭간골절적림상료효진행연구。방법선취2009년2월~2013년10월재아원채용PENA치료노년고골조륭간골절적환자36례，골절안AO분류표준，기중A1형14례，A2형16례，A3형6례。최후진행회고분석，주요분석기수술시간、술중실혈량、골절유합정황이급관관절공능회복정황。결과경과PFNA치료후，36례환자적고골조륭간골절균이체도림상유합，병차술후균미출현감염、내고정실패，야미출현고골두결혈배사급관내번기형등증상。병차，평개관관절공능적총우량솔체도료97.9%。결론노년환자고골조륭골절채용PFNA치료불단창상소、병차고정뢰은、환가이감소병발증적발생。괄합노년환자여대수술내수력교소적환자，병장기작위수선치료방안。
Objective Against proximal femoral FangXuan bladenail(PENA)clinical ef icacy in elderly patients with intertrochanteric of fracture were studied.Methods February 2009~October 2013 in our hospital PENA treatment of senile intertrochanteric fracture 36 patients And car ies on the classification by AO,including 14 cases of type A1,16 cases of type A2,6 cases of type A3,Final analysisMainly analyze the operation time,blood loss,fracture healing and hip function recovery.Results After PFNA treatment,36 cases of femoral intertrochanteric fracture have reached clinical healing,And there was no infection after operation,Internal fixation failure,no of the femoral head necrosis and the deformity of the hip.And the total excel ent rate of hip joint function was 97.9%. Conclusion PFNA treatment of intertrochanteric fracture in elderly patients is not only a minor trauma,And the fixation is stable,but also can reduce the incidence of complications.Suitable for elderly patients Patients with less tolerance for surgery,And treatit the first choice of treatment.
