目的了解轻度胃肠炎伴婴幼儿良性惊厥( BICE)的临床表现及预后。方法对2013年10月~2014年10月苏州大学附属儿童医院神经科病房住院的63例符合BICE诊断的患儿临床资料及出院随访结果，进行回顾性研究。结果63例患儿发病年龄12~31个月，平均年龄(18.24±4.73)个月；惊厥多发生在胃肠道症状出现后3d内，其中37例(58.7%)出现2次以上惊厥，丛集性发作多集中在24h之内。20例轮状病毒检测阳性。BICE多发于11~12月份。9例患儿出现发作间期脑电图异常，主要表现为枕部慢波增多。63例均行颅脑影像学检查，未见明显异常，并于发病后12个月电话随访，无惊厥发作。结论 BICE多发生在1~3岁幼儿，发病有明显季节性。1/2患儿出现2次以上惊厥，丛集性发作多集中在24h之内。轮状病毒为主要感染源。BICE预后良好，无需长期抗癫痫治疗。
목적료해경도위장염반영유인량성량궐( BICE)적림상표현급예후。방법대2013년10월~2014년10월소주대학부속인동의원신경과병방주원적63례부합BICE진단적환인림상자료급출원수방결과，진행회고성연구。결과63례환인발병년령12~31개월，평균년령(18.24±4.73)개월；량궐다발생재위장도증상출현후3d내，기중37례(58.7%)출현2차이상량궐，총집성발작다집중재24h지내。20례륜상병독검측양성。BICE다발우11~12월빈。9례환인출현발작간기뇌전도이상，주요표현위침부만파증다。63례균행로뇌영상학검사，미견명현이상，병우발병후12개월전화수방，무량궐발작。결론 BICE다발생재1~3세유인，발병유명현계절성。1/2환인출현2차이상량궐，총집성발작다집중재24h지내。륜상병독위주요감염원。BICE예후량호，무수장기항전간치료。
Objective To determine the clinical characteristics and recent prognosis of benign infantile convulsions associated with mild gastroenteritis (BICE). Methods Studied clinical features of 63 children with BICE who were hospitalized between october 2013 and october 2014 and then fol owed for at least 1 years' fol ow-ups in the children hospital of Soochow University. Results Age of onset of these 63 children ranged from 12 to 31 months with the average age of 18.24±4.73 months;the seizures may occur within the ?rst 3 days of gastroenteritis.37 patients (58.7%)had 2 or more seizures, which mostly recur ed within 24h. Rotavirus antigen was detected in the stool specimens of 20 patients. BICE occur ed dominantly in November and December. Interictal EEG of 9 children proved abnormal, which showed slow waves on occipital regions;Brain imaging studies were performed in 63 patients and revealed no obvious abnormalities. Al patients was carried out the fol ow-ups after the onset of the seizures by phone,and no children suf ered from recur ence of the seizures. Conclusion BICE showed a distinct seasonal distribution and happened in patients between one to three years old. About half of the patients suf ered two or more seizures which mainly reoccur ed within 24 hours. Rotavirus is the most common agent among the diar heal viruses that may cause BICE. Prognosis can have a good ef ect on patients with BICE,so it is not necessary for the children to receive long-term antiepileptic therapy.
