目的：观察吻合器痔上直肠黏膜环切（PPH）加小切口外剥内扎术治疗重度混合痔的临床疗效。方法随机选择2010年5月至2014年8月来我院进行重度混合痔治疗的患者300例，随机分为观察组与对照组各150例。观察组：行PPH加小切口外剥内扎术。对照组：对照组行常规外剥内扎术。观察两组患者手术相关指标、术后并发症和复发率。结果观察组的手术时间、术中出血量、住院时间和恢复工作时间明显少于对照组，且具有统计学差异（P＜0.05）。观察组并发症发生率明显低于对照组，且具有统计学差异（P＜0.05）。随访6个月，两组均无复发患者，两组的复发率无统计学差异（P＞0.05）。结论 PPH加小切口外剥内扎术对于重度混合痔具有良好的临床疗效。
목적：관찰문합기치상직장점막배절（PPH）가소절구외박내찰술치료중도혼합치적림상료효。방법수궤선택2010년5월지2014년8월래아원진행중도혼합치치료적환자300례，수궤분위관찰조여대조조각150례。관찰조：행PPH가소절구외박내찰술。대조조：대조조행상규외박내찰술。관찰량조환자수술상관지표、술후병발증화복발솔。결과관찰조적수술시간、술중출혈량、주원시간화회복공작시간명현소우대조조，차구유통계학차이（P＜0.05）。관찰조병발증발생솔명현저우대조조，차구유통계학차이（P＜0.05）。수방6개월，량조균무복발환자，량조적복발솔무통계학차이（P＞0.05）。결론 PPH가소절구외박내찰술대우중도혼합치구유량호적림상료효。
ObjectiveTo observe the clinical curative effect of treatment of severe mixed hemorrhoids by procedure for prolapse and hemorrhoids(PPH) and small incision Milligan-Morgan.Methods 300 cases with severe mixed hemorrhoids, which admitted in our hospital during May 2010 - August 2014, divided into the observation group and the control group, each group of 150 cases. The observation group was given PPH and small incision Milligan-Morgan. The control group was given conventional Milligan-Morgan. The operation related indicators, postoperative complications and recurrence rate of the two groups were compared.Results Operation time, intraoperative blood loss, hospital stay and recovery time of the observation group was significantly less than the control group, the difference was significant statistically significant(P<0.05). The complication incidence of observation group was obviously lower than the control group, the difference was significant statistically significant(P<0.05). They were followed up for 6 months, two groups of patients had no recurrence, and the recurrence rate of the two groups has no statistical difference(P>0.05).Conclusion PPH and small Milligan-Morgan has a good clinical curative effect for severe mixed hemorrhoids.
