目的：研究米卡芬净预防造血干细胞移植（HSCT）后早期真菌感染的临床效果和安全性。方法收集我院血液科2009年2月至2015年1月HSCT33例，8例移植前有肺部真菌感染史，从预处理开始使用米卡芬净50mg/d，其余患者从中性粒细胞缺乏时使用50~100mg/d。结果33例患者中1例发生了眶周感染、中枢神经系统真菌感染，在米卡芬净使用过程中未观察到不良反应发生，也未因此而调整环孢素A浓度，到随访结束时，26例存活。结论米卡芬净对预防造血干细胞移植早期真菌感染是安全、有效的，不影响环孢素A血药浓度。
목적：연구미잡분정예방조혈간세포이식（HSCT）후조기진균감염적림상효과화안전성。방법수집아원혈액과2009년2월지2015년1월HSCT33례，8례이식전유폐부진균감염사，종예처리개시사용미잡분정50mg/d，기여환자종중성립세포결핍시사용50~100mg/d。결과33례환자중1례발생료광주감염、중추신경계통진균감염，재미잡분정사용과정중미관찰도불량반응발생，야미인차이조정배포소A농도，도수방결속시，26례존활。결론미잡분정대예방조혈간세포이식조기진균감염시안전、유효적，불영향배포소A혈약농도。
Objective To explore the efifciency and safety of micafungin prevented early mycotic infection after hematopoietic stem cell transplantation. Methods??33patients?who?receiveed?micafungin?prevented?mycotic?infection?after?hematopoietic?stem?cell?transplantation?were?analyzed?retrospectively,?8 patients were pulmonary fungal infection before HSCT, micafungin 50 mg/day were injected beginning from pretreatment, other patients were injected micafungin?50-100?mg/day?beginning?from?agranulocytosis.?Results?one?case?occurred?mycotic?infection?around?the?eyes?and?central?nervous?system?in?all?patients, no toxic effect were observed during the clinical observation, also blood concentration of ciclosporin A wasnot adjusted, ifnally 26 patients survival. Conclusion??Micafungin?prevented?early?mycotic?infection?after?hematopoietic?stem?cell?transplantation?is?safety?and?effective,?blood?concentration?of?ciclosporin?A?no?change.
