目的：观察经支气管镜注入立止血治疗镜下检查出血及大咯血的临床效果。方法抽取2012年2月至2014年4月在我院经支气管镜检查发现病灶或出血患者120例，先以1∶10000肾上腺素2 mL注入出血部位或病灶病位，后以标准活钳对支气管黏膜进行活检与刷检，对部分患者的肺组织进行活检与刷检。后对出血部位进行负压吸引，并注入冰冻盐水与肾上腺素的混合液，数秒后吸除积血，并查看是否仍有出血情况。若仍有血液涌出，则需行立止血处理，给药方法为经吸引口用长硅胶管直接注洒立止血1~2 ku，并以支气管镜进行观察，直至出血停止再将支气管镜取出。结果120例出血患者在使用立止血后出血迅速停止，无不良事件发生。结论对于镜下检查出血及大咯血患者以立止血进行止血治疗效果显著，能在短时间内达到止血的目的，预防不良事件发生。
목적：관찰경지기관경주입립지혈치료경하검사출혈급대각혈적림상효과。방법추취2012년2월지2014년4월재아원경지기관경검사발현병조혹출혈환자120례，선이1∶10000신상선소2 mL주입출혈부위혹병조병위，후이표준활겸대지기관점막진행활검여쇄검，대부분환자적폐조직진행활검여쇄검。후대출혈부위진행부압흡인，병주입빙동염수여신상선소적혼합액，수초후흡제적혈，병사간시부잉유출혈정황。약잉유혈액용출，칙수행립지혈처리，급약방법위경흡인구용장규효관직접주쇄립지혈1~2 ku，병이지기관경진행관찰，직지출혈정지재장지기관경취출。결과120례출혈환자재사용립지혈후출혈신속정지，무불량사건발생。결론대우경하검사출혈급대각혈환자이립지혈진행지혈치료효과현저，능재단시간내체도지혈적목적，예방불량사건발생。
Objective To observed the clinical effect of Reptilase for bleeding and big haemoptysis under bronchoscope by Bronchoscopy injection.Methods 120 cases of patients were selected, who were found lesions or hemorrhage under bronchoscope, they were all treated with 2 mL adrenaline (1∶10000) by bronchoscopy injection and followed by bronchial mucosal biopsies and brush by standard live pliers, and then Pressure suction bleeding, injected mixture of adrenalineznd frozen brine, Gettering hemorrhage after a few seconds, If there was still bleeding, it needs 1-2 ku Reptilase until the bleeding stop, drug delivery method was direct injection with silicone tube through Attract mouth.Results All patients stopped bleeding quickly after using reptilase, no adverse events.Conclusion For treatment of patients with hemorrhage and massive hemoptysis, Reptilase has significant curative effect and could hemostasis in a short period of time and prevent adverse events.
