目的：分析病例和复习文献以提高对不典型肝脓肿的临床表现、病理特征、诊断和治疗的认识。方法回顾性分析1例不典型肝脓肿的临床资料并复习相关文献。结果该例肝脓肿患者缺乏典型临床表现，而且肝脓肿液化不明显，虽然病灶较大，但是积极抗感染治疗后，病灶迅速缩小直至消失，治疗难度不大。结论不典型肝脓肿由于影像学特征与肝癌相似，容易误诊，但是如果有高热症状，抗感染治疗有效，首先考虑感染因素。可动态观察病灶影像学变化，必要时行肝脏穿刺明确诊断。

Objective To improve understanding of the clinical and pathological feature ,diagnosis and treatment of atypical liver abscess through analyzing cases and literature review .Methods One case of atypical liver abscess was studied retrospectively of clinical data , and related literatures were reviewed. Results Atypical liver abscess was short of typical clinical manifestations and wasn′t liquefied obviously. Though it had larger lesions,after active anti-infection treatment,the lesions rapidly shrank until it disap-peared.The treatment was not difficult.Conclusion Atypical liver abscess is easily misdiagnosed because the imaging features are similar to liver cancer.If there is fever symptoms and the anti-infection is effective, the infection factor should be considered first .Dynamic imaging changes could provide valuable information , and if necessary,liver biopsy can be done to confirm the diagnosis.
