目的:探讨补肾健脾活血解毒法对早期糖尿病肾病模型大鼠单核细胞趋化蛋白-1(MCP-1)及硫氧还原蛋白相互作用蛋白(Txnip)表达的影响。方法采用STZ适量腹腔注射方法制造早期糖尿病肾病大鼠模型,将40只造模成功大鼠随机分为模型组、西药组、中药小剂量组、中药中剂量组、中药大剂量组,每组8只。另取8只健康大鼠作为空白组。模型组及空白组给予生理盐水灌胃,中药大、中、小剂量组分别给予按补肾健脾活血解毒法立方的生药浓度5 mg/L、2.5 mg/L、1.25 mg/L煎煮的中药药液灌胃,西药组给予蒙诺混悬液灌胃。干预8周后,观察各组大鼠肾脏组织MCP-1免疫组化及Txnip mRNA的表达情况。结果中药大、中、小剂量组大鼠的体质量均高于模型组,肾质量均轻于模型组,尿量、24 h尿蛋白定量及血糖、血脂水平,MCP-1阳性积分及Txnip mRNA表达量均明显低于模型组,且中药大剂量组降低最为明显,差异均有统计学意义(P均<0.05)。结论以补肾健脾活血解毒法立方的中药可以降糖、调脂,减少24 h尿蛋白定量及肾脏MCP-1免疫组化、Txnip mRNA的表达,从而保护肾功能,值得进一步研究。

목적:탐토보신건비활혈해독법대조기당뇨병신병모형대서단핵세포추화단백-1(MCP-1)급류양환원단백상호작용단백(Txnip)표체적영향。방법채용STZ괄량복강주사방법제조조기당뇨병신병대서모형,장40지조모성공대서수궤분위모형조、서약조、중약소제량조、중약중제량조、중약대제량조,매조8지。령취8지건강대서작위공백조。모형조급공백조급여생리염수관위,중약대、중、소제량조분별급여안보신건비활혈해독법립방적생약농도5 mg/L、2.5 mg/L、1.25 mg/L전자적중약약액관위,서약조급여몽낙혼현액관위。간예8주후,관찰각조대서신장조직MCP-1면역조화급Txnip mRNA적표체정황。결과중약대、중、소제량조대서적체질량균고우모형조,신질량균경우모형조,뇨량、24 h뇨단백정량급혈당、혈지수평,MCP-1양성적분급Txnip mRNA표체량균명현저우모형조,차중약대제량조강저최위명현,차이균유통계학의의(P균<0.05)。결론이보신건비활혈해독법립방적중약가이강당、조지,감소24 h뇨단백정량급신장MCP-1면역조화、Txnip mRNA적표체,종이보호신공능,치득진일보연구。
Objective It is to explore the effect of the method of invigorating spleen and kidney , detoxification and vigora-tion of blood circulation on MCP -1 and Txinp expression in rat models of early diabetic nephropathy .Methods The model of early diabetic nephropathy were made by intraperitoneal injection of STZ .The model rats were randomly divided into 5 groups (n=8):model group, low-dose TCM group, middle -dose TCM group, high-dose TCM group and fosinopril -sodium group.Another 8 healthy rats were selected as blank group .The model group and blank group were given normal saline , high, middle and low dosage TCM groups were given drugs which can invigorating spleen and kidney , activating blood circulation and removing toxics at the dosage of 5 mg/L, 2.5 mg/L and 1.25 mg/L respectively, fosinopril-sodium group was given fosi-nopril-sodium liquid .After 8 weeks of drug intervention , the expression of MCP-1 and Txinp in the rats in each group were measured by immunohistochemical method and PCR respectively .Results The weight of the rats were higher and urine output , quantitative of ptoteinuria in 24 hour, blood sugar, blood lipid level, MCP-1 positive scores and Txnip expression were lower in every TCM group than those in model group , and the decrease in high dose TCM group was the most obviously , the differ-ences were all significant(P all<0.05).Conclusion The method of invigorating spleen and kidney , detoxification and vigora-tion of blood circulation can protect renal function by the way of decreasing the urine output , quantitative of ptoteinuria in 24 hour, blood sugar, blood lipid and inhibiting MCP -1 and Txnip expression, it is worthy further studying.
