目的:研究维持性血液透析( MHD)患者血液透析( HD)前后Tp-e/QT的变化及其影响因素。方法60例维持性血液透析患者分别于透析前及透析后立即记录十二导联同步心电图,计算Tp-e及Tp-e/QT值,同时采取外周血测定生化指标,记录收缩压、舒张压、超滤脱水量( UF/W )等生理指标。结果 MHD患者HD前Tp-e及Tp-e/QT值明显高于正常对照组(P<0.01),HD后Tp-e及Tp-e/QT进一步增高(P<0.01);回归分析表明,Tp-e/QT与血清HCO3-呈正相关(r=0.39,P<0.0.1),与UF/W呈正相关(r=0.41,P<0.0.1);多元回归分析显示,血清HCO3-、UF/W是HD后Tp-e/QT增高的独立影响因素。结论 MHD患者Tp-e/QT增高,透析使Tp-e/QT进一步增高,其发生机制可能与透析脱水及快速纠正酸中毒有关。

목적:연구유지성혈액투석( MHD)환자혈액투석( HD)전후Tp-e/QT적변화급기영향인소。방법60례유지성혈액투석환자분별우투석전급투석후립즉기록십이도련동보심전도,계산Tp-e급Tp-e/QT치,동시채취외주혈측정생화지표,기록수축압、서장압、초려탈수량( UF/W )등생리지표。결과 MHD환자HD전Tp-e급Tp-e/QT치명현고우정상대조조(P<0.01),HD후Tp-e급Tp-e/QT진일보증고(P<0.01);회귀분석표명,Tp-e/QT여혈청HCO3-정정상관(r=0.39,P<0.0.1),여UF/W정정상관(r=0.41,P<0.0.1);다원회귀분석현시,혈청HCO3-、UF/W시HD후Tp-e/QT증고적독립영향인소。결론 MHD환자Tp-e/QT증고,투석사Tp-e/QT진일보증고,기발생궤제가능여투석탈수급쾌속규정산중독유관。
Objective To study the changes of Tp-e/QT in patients with maintenance hemodialysis (MHD) before and after hemodialysis (HD) and its influencing factors.Methods 60 cases in maintenance hemodialysis patients were in pre dialysis and post dialysis immediately record of 12 lead synchronous ECG and Tp-e and Tp-e/Qt value is calculated , also take peripheral blood biochemical parameters were measured , systolic blood pressure, diastolic blood pressure, and ultrafiltration de physiological index of water (UF/W).Results The MHD patients with HD Tp-e and Tp-e/Qt value was significantly higher than that in normal control group (P<0.01), HD Tp-e and Tp-e/Qt further increased (P <0.01); regression analysis showed, Tp-e/QT and serum HCO3-were positively correlated (r=0.39, P<0.0.1), positively with UF/W correlation (r=0.41, P<0.0.1);multiple regression analysis showed that serum HCO3-, UF/W Tp-e/Qt increased post HD independent factors.Conclusions The MHD of patients with Tp-e/QT increased, and dialysis increased the Tp-e/QT.The mechanism may be related to the dehydration and rapid correction of acidosis .
