目的:研究高压油漆注射伤新西兰大白兔血管神经组织病理变化及肢体血循环变化的规律,为临床治疗该类损伤提供病理学依据。方法取健康雄性新西兰大白兔,耳缘静脉麻醉后用高压油漆喷枪向新西兰大白兔的腹股沟区喷射工业用油漆,注射后8h内每隔1 h切取新西兰大白兔的股动脉、股神经、股静脉制作切片,HE染色后分别在光学显微镜及扫描电镜下观察组织病理学变化,并肉眼观察各时点新西兰大白兔伤肢肢体血液循环情况。结果随着时间的推移,新西兰大白兔的股动脉搏动逐渐减弱消失,肢体远端毛细血管回流消失。光镜下股静脉出现静脉炎表现;股动脉内膜的连续性发生破坏,内有血栓形成;股神经的外膜连续性发生破坏,外膜周围附着有大量炎性细胞。电镜下发现血管内膜破坏,肌层细胞向血管内膜突入,纤维条索形成,神经髓鞘松弛排列紊乱,线粒体空泡形成。高压油漆喷枪注射伤后8 h,兔的股动脉、股静脉、股神经出现了不可逆性严重损伤。结论对于高压油漆注射伤这类严重的复合性损伤,应尽可能在伤后8h内进行彻底手术清创,避免重要血管神经出现不可逆性、灾难性损伤,最大限度降低损伤程度。

목적:연구고압유칠주사상신서란대백토혈관신경조직병리변화급지체혈순배변화적규률,위림상치료해류손상제공병이학의거。방법취건강웅성신서란대백토,이연정맥마취후용고압유칠분창향신서란대백토적복고구구분사공업용유칠,주사후8h내매격1 h절취신서란대백토적고동맥、고신경、고정맥제작절편,HE염색후분별재광학현미경급소묘전경하관찰조직병이학변화,병육안관찰각시점신서란대백토상지지체혈액순배정황。결과수착시간적추이,신서란대백토적고동맥박동축점감약소실,지체원단모세혈관회류소실。광경하고정맥출현정맥염표현;고동맥내막적련속성발생파배,내유혈전형성;고신경적외막련속성발생파배,외막주위부착유대량염성세포。전경하발현혈관내막파배,기층세포향혈관내막돌입,섬유조색형성,신경수초송이배렬문란,선립체공포형성。고압유칠분창주사상후8 h,토적고동맥、고정맥、고신경출현료불가역성엄중손상。결론대우고압유칠주사상저류엄중적복합성손상,응진가능재상후8h내진행철저수술청창,피면중요혈관신경출현불가역성、재난성손상,최대한도강저손상정도。
Objective It is to summarize the high pressure paint injection injury in New Zealand rabbits vessels and nerves pathological and body blood circulation organization change rules , in order to provides the basis for clinical pathology laboratory to the clinical treatment and early debridement put forward effective time point .Methods Health male New Zealand big white rabbits were selected and were injected industrial paint to the groin area jet using high pressure airbrush, after the injection( 0-8 hours) per hour the white rabbit femoral artery , the nerve, femoral venous were cut off to make slice for HE staining to observe histopathological changes under light microscopy and scanning electron microscope , and blood circulation of injuried limbs were observed by eyes .Results With the time going , pulses of femoral artery of the rabbits weakened gradually or disap-peared , limbs capillary backflow disappeared .By light microscopy femoral venous appear phlebitis performance , the lining of the femoral artery continuity happen damage , with thrombosis .Shares of nerve continuity , the outer membrane happened de-struction, around the outer membrane attached with a lot of inflammatory cells .Electron microscopy revealed tunicaintimal damage , muscle cells to vascular intimal entry , fiber stripe formation , myelin relaxation derangement , mitochondrial vacuol-ization.Conclusion For the serious compound damage such as high pressure injection injury by airbrush , completed debride-ment within 8h after damage is needed as soon as possible , in order to avoid irreversible serious damage of important blood ves-sel and nerve , and furthest relieve injury degree .
