目的:探讨圆锥角膜和近视人群波前像差及屈光参数的差异性,为临床实践提供一定的依据方法随机收集2012年5月~2015年5月,在我院眼科门诊就诊的16例(32眼)的双眼原发性圆锥角膜病例和16例(32眼)双眼近视的病例,圆锥角膜患者为A组,近视患者为B组。检查者测量病例受试者双眼像差,双眼屈光不正度数;眼轴长AL,前房深度ACD,角膜曲率。运用SPSS17.0统计分析软件分别进行圆锥角膜和近视人群高阶像差对等效球镜SE、眼轴AL、角膜曲率及前房深度ACD的相关分析,并比较二者的差异性。结果首先F检验显示两组的数据方差齐性(>0.05),运用独立样本T检验结果显示:圆锥角膜和近视的患者相比较,其一些高阶波前像差值(球差、彗差、四叶草)、角膜曲率及前房深度都会增大。Pearson双变量相关分析显示:在圆锥角膜病例中,年龄和眼轴长、眼轴长和前房深度、角膜曲率和年龄、角膜曲率和前房深度均显示显著性正相关,年龄和等效球镜、等效球镜和眼轴长、等效球镜和前房深度显著性负相关;等效球镜和球差、前方深度和慧差、前方深度和球差、角膜曲率和球差、角膜曲率和慧差、角膜曲率和四叶草均显示显著性正相关,年龄和慧差呈显著性负相关。而在近视人群中,年龄和眼轴长、眼轴长和前房深度显著性正相关,年龄和等效球镜、等效球镜和眼轴长、等效球镜和前房深度显著性负相关;等效球镜和球差、前房深度和彗差、前房深度和球差显著性正相关。结论在圆锥角膜及近视病例研究都显示:等效球镜和年龄,眼轴长和前房深度,等效球镜以及前房深度和高阶波前像差(球差和慧差)有密切相关性;圆锥角膜病例中,年龄、角膜曲率对像差的影响显著。研究结果亦显示,随着等效球镜向近视方向发展,球差值会增大;慧差和眼轴长及前房深度均呈正相关。

Objective To explore the keratoconus and myopic crowd of the dif erence of wavefront aber ation and refractive parameters provide a basis for clinical practice.Methods 16 patients af ected by keratoconus (32 eyes)and 16 patients with myopes (32 eyes)were recruited from outpatient clinic randomly.Group A are keratoconus,and Group B are myopes.wavefront aberrations,degrees of refraction was measured using auto refractor,axial length (AL)and anterior chamber depth (ACD)and corneal curvature were measured by the same doctor.SPSS software package 17.0 were applied to perform the data analysis.Results Data showed that some higher order aber ations (sphere-aber ation, coma-aber ation and quadrafoil)anterior chamber depth and corneal curvature wil increase in patients with keratoconus compared to myopes (the results were analyzed with the Matching T test)Pearson test in patients with keratoconus (Group A)statistics showed that Age and AL,AL and ACD, corneal curvature and Age,corneal curvature and ACD had positive cor elations,Age and SE,SE and AL,SE and ACD had negative cor elations;SE and sphere-aber ation,ACD and coma-aberration, ACD and sphere-aberration,corneal curvature and sphere-aber ation,corneal curvature and coma-aber ation,corneal curvature and quadrafoil had positive cor elations,Age and coma-aber ation had negative cor elations;Pearson test in patients with myopes (Group B)Age and AL,AL and ACD had positive correlations,Age and SE,SE and AL,SE and ACD had negative cor elations;SE and sphere-aber ation,ACD and coma-aber ation,ACD and sphere-aber ation had positive cor elations .Conclusion Both keratoconus and myopes,SE and Age,AL,SE,higher order aber ations (sphere-aberration, coma-aber ation)and ACD were wel cor elated.In patients with keratoconus,Age and corneal curvature had significant influence in ocular aber ations.Sphere-aber ation was increased as SE developed to myopia.Coma-aber ation,AL and ACD have-positive-cor elations.
