目的:观察急性脑梗死患者血浆B型脑钠肽(BNP)的浓度变化,初步探讨血浆BNP与脑梗死部位、面积及预后的关系。方法选取我院急性脑梗死患者60例,在入院后24 h内采用ELISA法测定血浆BNP ,并与健康体检者40例对比。所有患者均记录入院后梗死部位、面积及终点事件。结果脑梗死患者血浆BNP水平显著高于对照组(P<0.01);其中丘脑及基底节区梗死血浆BNP水平显著高于其他部位梗死(P<0.01);大面积脑梗死组血浆BNP水平显著高于中等面积脑梗死组(P<0.05)。预后不良组血浆BNP水平显著高于预后良好组(P<0.01)。结论急性脑梗死患者BNP水平较健康体检者明显升高,丘脑及基底节梗死;大、中等面积梗死及不良预后的患者血浆BNP明显升高。血浆BNP有望成为判断脑梗死预后指标之一。

목적:관찰급성뇌경사환자혈장B형뇌납태(BNP)적농도변화,초보탐토혈장BNP여뇌경사부위、면적급예후적관계。방법선취아원급성뇌경사환자60례,재입원후24 h내채용ELISA법측정혈장BNP ,병여건강체검자40례대비。소유환자균기록입원후경사부위、면적급종점사건。결과뇌경사환자혈장BNP수평현저고우대조조(P<0.01);기중구뇌급기저절구경사혈장BNP수평현저고우기타부위경사(P<0.01);대면적뇌경사조혈장BNP수평현저고우중등면적뇌경사조(P<0.05)。예후불량조혈장BNP수평현저고우예후량호조(P<0.01)。결론급성뇌경사환자BNP수평교건강체검자명현승고,구뇌급기저절경사;대、중등면적경사급불량예후적환자혈장BNP명현승고。혈장BNP유망성위판단뇌경사예후지표지일。
Objective To study the changes of brain natriuretic peptide in patients with acute cerebral infarction.Methods Sixty patients with acute cerebral infarction and 40 healthy volunteer were selected into the study. Their serum levels of BNP were detected by ELISA in 24 hours after admission. The areas of infarction ,size of infarct and endpoint events of all the pa‐tients were recorded after admission. Results The level of BNP in acute cerebral infarction group was significantly higher than that in control group (P<0.01). The level of BNP in thalamus and basal nucleus infarction was significantly higher than that in other parts of infarction (P<0.01). The serum level of BNP in patients with massive cerebral infarction was significantly higher than that in patients with non‐massive cerebral infarction (P<0.05). BNP levels in the poor prognosis group were sig‐nificantly higher than those of the good outcome group (P<0.01). Conclusion The level of BNP is significantly higher in pa‐tients with acute cerebral infarction. The level of BNP is closely related to the cerebral infarction location ,the cerebral infarc‐tion area and cerebral infarction prognosis. Therefore ,the plasma levels of BNP may become one of the important and quick in‐dexes to monitor the changes of condition.
