目的:探讨糖化血红蛋白(HbA1c)及血脂与急性缺血性脑卒中伴脑微出血(CMBs)的相关性。方法选择2013‐02—2014‐02收治的120例急性缺血性脑卒中患者,根据是否伴脑微出血分为观察组(伴 CMBs)65例,对照组(无CMBs)55例,对比2组的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、高密度脂蛋白(HDL)、APO‐A1、APO‐B和HbA1c等生化指标含量,并进行相关性回归分析。结果观察组总胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白、APO‐A1、APO‐B、HbA1c水平均与对照组有所差异,但仅 TC及 HbA1c差异明显,即 TC水平低于对照组,HbA1c水平高于对照组,差异有统计学意义(P<0.05);伴脑微出血的急性缺血性脑卒中患者,SWI检查显示CMBs位置主要位于基底节区,占50.8%,患者年龄主要分布在65~80岁(72.3%)阶段,明显高于45~65岁阶段,差异有统计学意义( P<0.05)。结论糖化血红蛋白及胆固醇水平与急性缺血性脑卒患者发生脑微出血存在一定的相关性,两者是产生脑微出血现象的危险因素。

목적:탐토당화혈홍단백(HbA1c)급혈지여급성결혈성뇌졸중반뇌미출혈(CMBs)적상관성。방법선택2013‐02—2014‐02수치적120례급성결혈성뇌졸중환자,근거시부반뇌미출혈분위관찰조(반 CMBs)65례,대조조(무CMBs)55례,대비2조적총담고순(TC)、감유삼지(TG)、저밀도지단백(LDL)、고밀도지단백(HDL)、APO‐A1、APO‐B화HbA1c등생화지표함량,병진행상관성회귀분석。결과관찰조총담고순、감유삼지、저밀도지단백、고밀도지단백、APO‐A1、APO‐B、HbA1c수평균여대조조유소차이,단부 TC급 HbA1c차이명현,즉 TC수평저우대조조,HbA1c수평고우대조조,차이유통계학의의(P<0.05);반뇌미출혈적급성결혈성뇌졸중환자,SWI검사현시CMBs위치주요위우기저절구,점50.8%,환자년령주요분포재65~80세(72.3%)계단,명현고우45~65세계단,차이유통계학의의( P<0.05)。결론당화혈홍단백급담고순수평여급성결혈성뇌졸환자발생뇌미출혈존재일정적상관성,량자시산생뇌미출혈현상적위험인소。
Objective To analyze the correlation of brain microhemorrhage glycosylated hemoglobin and blood lipids asso‐ciated with acute ischemic stroke.Methods One hundred and twenty acute ischemic stroke patients from February 2013 to Feb‐ruary 2014 were selected ,and were divided into trail group (with CMBs ,65 cases) and control group (without CMBs ,55 ca‐ses). The level of total cholesterol (TC) ,triglyceride (TG) ,low density lipoprotein (LDL) ,high density lipoprotein (HDL) , APO‐A1 ,APO‐B and HbA1c and other biochemical markers of two group were compared ,and they were analyzed by correla‐tion regression.Results HbA1c level of trail group was higher than that of control group (P<0.05) ,TC level of trail group was lower than that of control group (P<0.05);SWI displayed that CMBs of trail lied in basal ganglia mostly ,and patients in trail group age were 65‐80 years mostly ,which were higher than the patients with 65‐80 years old (P<0.05).Conclusion Brain microhemorrhage certain correlation between glycated hemoglobin and cholesterol levels and stroke in patients with acute ischemic brain occurs ,both of which are risk factors for cerebral microhemorrhage produce phenomena.
