目的 观察臭氧水关节腔注射对兔膝骨关节炎软骨的病理变化及臭氧水对兔膝骨关节炎软骨细胞中血管内皮生长因子(VEGF)和肿瘤坏死因子-α(TNF-α)的影响.方法 将40只新西兰大白兔随机分为4组:正常组(A)、模型组(B)、生理盐水组(C)、臭氧水组(D),每组10只.除正常组外,其余3组均采用关节腔注射木瓜蛋白酶溶液造模.造模成功后,生理盐水组注射0.9%NaCl 2 ml,臭氧水组注射浓度为20 mg/L臭氧水2ml,每周注射1次,共3次.末次注射1周后处死全部兔,观察4组兔膝关节软骨的病理变化及软骨中VEGF和TNF-α的表达水平.结果 A组为正常关节软骨;B、C组表现为膝骨关节炎病理损害改变;D组软骨病理损害明显减轻.软骨细胞中VEGF、TNF-α的表达:与A组[(61.98±16.07)、(29.16 ±8.87) ng/L]比较,B组VEGF、TNF-α的表达水平[(280.04±36.80)、(135.05±16.87) ng/L]明显升高(P<0.05);与B组比较,C组[(276.74±28.73)、(130.31±12.10) ng/L]和D组[(187.21±24.49)、(83.76±17.52) ng/L]中VEGF、TNF-o的表达均降低,D组降低显著(P<0.05);D组软骨细胞中VEGF、TNF-α浓度明显低于C组(P<0.05).结论 臭氧水关节腔注射能够有效抑制软骨的病理损害,降低软骨细胞中VEGF和TNF-α的表达.

Objective To investigate the effects of intra-articular injection of ozonated water on vascular endothelial growth factor (VEGF) and tumor necrosis factor-α (TNF-α) in rabbit knee osteoarthritic cartilage and chondrocytes.Methods Forty New Zealand white rabbits were randomly divided into four groups : normal group (A), model group (B), saline group (C), and ozonated water group (D) (n =10 each).Except the normal group, the rest three groups were given intra-articular injection of papain solution for modeling.After the model was established, saline group was injected with 0.9% NaCl 2 ml, and ozonated water group ozonated water 2 ml, once a week for 3 times.One week after the last injection, all the rabbits were sacrificed, and pathological changes of rabbit knee joint cartilage and the expression of VEGF and TNF-α were compared among four groups.Results Normal articular cartilage was seen in group A.In groups B and group C, pathological lesions of knee osteoarthritis change were seen.In group D, pathological damage was significantly reduced.As compared with group A, the expression levels of VEGF and TNF-α in chondrocytes in group B were significantly increased [(61.98 ± 16.07) and (29.16±8.87) ng/L vs.(280.04±36.80) and (135.05 ± 16.87) ng/L] (P<0.05).As compared with group B, the expression of VEGF and TNF-α in chondrocytes in group C [(276.74 ± 28.73) and (130.31 ± 12.10) ng/L] and group D [(187.21 ± 24.49) and (83.76 ± 17.52) ng/L] were significantly reduced (P < 0.05).The expression of VEGF and TNF-α in chondrocytes in group D was significantly lower than in group C (P < 0.05).Conclusion Intra-articular injection of ozonated water can inhibit the pathological cartilage damage and reduce the expression of VEGF and TNF-α in chondrocytes.
